0049389999 2014-06-24 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac #72. They walked the rig from the #8H and spudded the 17.5 surface hole of this well at 7:30am. The surface casing cement job will be later this evening. No violations.
0447385831 2014-06-24 Casing String Witnessed No Violations I was on location to witness the 13.375 surface casing cement job. Nomac #72 drilled a 17.5 hole to a depth of 833 and Wyoming Casing ran 54.5lb/ft. casing to a depth of 822. Schlumberger cemented the casing with 159 barrels of Class A cement weighing 15.6lbs/gal. 33 barrels of slurry circulated to surface during the job. See cement report for more details on this job. No violations.
0646359835 2014-07-07 Drill / Deepen / Reopen No Violations I was on location to inspect the rig move and collect the production casing job logs from 7/6/14. They currently have the derrick laid over with the cranes. The holiday weekend has delayed moving the permitted loads to the Forcier Pad. No violations.
0666070813 2015-09-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of two 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0800853700 2016-03-10 Preliminary Restoration No Violations I was onsite to conduct a preliminary restoration inspection. There are four wells on the pad. The pad is shared between the Kruprzak #3H (22212-

There are concrete jersey barriers protecting the wellhead and production equipment from vehicles. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

1078514018 2017-09-06 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #6H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 2- 400 bbl production tanks; both labelled as containing produced water. 4 Producing wells 4 Sand separators 4 Gas processing units

2085687510 2014-07-02 Drill / Deepen / Reopen No Violations I was on location to check the drilling progress of Nomac #72. They are currently drilling the 8.5 lateral at a total measured depth of 19,508 with 1,200 left to drill. The location is in good condition and all drilling equipment is on containment. No violations.
