0012130661 2014-05-22 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #72. The rig had drilled the lateral to a total measured depth of 18,819 and Wyoming Casing was currently running 5.5 casing into the hole. I inspected the casing and found it was new, P-110 weighing 20lbs/ft. They should land and cement the casing later this evening. No violations.
0032626194 2014-05-15 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #72. They drilled the 8.75 hole to the mud up point of 6910 and switched too the 8.5 bit. They are currently drilling the curve at a depth of 7600. The location is in good condition and all drilling equipment is on containment. No violations.
0951390016 2014-05-27 Drill / Deepen / Reopen No Violations I was on location to check the status of moving Nomac Rig #72 to the Kruprzak #10H. The rig and derrick have been moved. The company trailers were still on location and I picked up the paperwork from the 5.5 production casing cement job. No violations.
1058505771 2014-05-12 Casing String Witnessed No Violations I was on location at 2:45pm to witness the cementing of the 9.625 intermediate casing. Nomac Rig #72 drilled a 12.25 hole to a depth of 2,275 and Wyoming Casing was in the process of running pipe to a depth of 2,263. I received an emergency call for a lightning strike at a tank battery in Brown Township and left location at 3:30pm. I returned to location at 5:45pm and Schlumberger was pumping cement. They pumped 178 barrels of slurry, dropped the plug, and displaced the plug to the collar. The plug landed at 900psi and the pressure was increased to 1400psi and held for 5 minutes. The pressure was released and the floats held. 25 barrels of cement, scaled at 15.5lbs/gal, circulated to surface. No violations. See casing ticket for more details on this job.
1105106537 2015-12-28 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1434383656 2015-04-28 Preliminary Restoration No Violations I meet with the Federal EPA at this location. They performed an Air Quality Inspection of the well and tank battery. I performed a status check of the tank battery and well head. ID signs are posted. No violations were noted at this time.
1790826868 2016-02-19 Preliminary Restoration No Violations I was on location for a status check. My inspection found the access road gate is open and the access road is in good repair. Identification signs are posted at the access road entrance, well and storage tanks. The pad is fenced and all man doors were locked. The #10H is in production. The well is equipped with a Christmas tree valve assembly. There is a steel dike around the storage tanks.

The equipment on the pad consists of the following: 6 Storage tanks 3 Sand separators 3 Gas processing units (GPU) 1 Vertical separator 1 Vapor recovery unit (VRU) 1 Heat treater 1 Thermal electric generator (TEG) 3 Production wells on the pad Site fenced and man doors locked
