--Tex Pumping LLC was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement slurry was mudscaled and O-Tex pumped 148.6 bbls of cement slurry at 15.8 ppg. The plug was displaced with 133 bbls of water as measured by the tanks on the pump truck. The plug bumped the float collar at 10:35pm. Pressure at plug bump was 850 psi. The pressure was increased to 1552 psi, and held for five minutes. Pressure held. The float held when the pressure was released, and 1.0 bbl of water bled back. 33 20 bbls of cement returned to the surface. Cement returns were mudscaled at 15.5 ppg.

1643273563 2014-07-03 Casing String Witnessed No Violations

-Tex Pumping LLC was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement slurry was mudscaled, and O-Tex pumped 85.5 bbls of cement slurry at 15.6 pounds per gallon (ppg). The plug was displaced with 60 bbls of water, as measured by the tanks on the pump truck. The plug bumped the float collar at 11:22pm on 07/03/2014. Schlumberger bumped the plug at 171 psi, and increased the pressure on the casing to 892 psi, and held it for five minutes. When the pressure was released, approximately 0.5 bbl of water flowed back, and the float held. 14 bbls of cement slurry returned to surface.

1704129007 2015-09-14 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1934623879 2014-06-26 Drill / Deepen / Reopen No Violations Express Energy drilled a 26 hole to a total depth of 120 and ran 120 of 20 welded 94# conductor casing. Express Energy pumped 9 barrels of fresh water followed by 230 sacks of class A cement. They displaced with 39.07 barrels of fresh water. There was 17 barrels of cement returned to surface. Job completed.
