0714023409 2015-03-02 Production Wells No Violations I inspected this location to assist Supervisor Tom Hill in following up on Complaint #4948 concerning odors possibly emanating from nearby producing operations. This well pad is 0.50 miles southwest of the complainants residence. I arrived on location and met with the well pumper, Kyle Bardon, who was monitoring and adjusting the production equipment. I spoke with Kyle and he informed me that they began turning the wells on 10 days ago. There are 3 wells beginning to produce on this pad: Permit #s 22562, 22577, & 22578. Each well is equipped with a line heater and produce through a sand separator to a fire-heated, gas production unit (GPU). The wells produce to a diked tank battery that contains six (6) 400 barrel tanks. The tanks are plumbed to a functioning vapor recovery unit (VRU). I could find no evidence of any odors being released that were described in the complaint. The only source I did find was from GPUs heat vent stack and produced a minor odor like new machinery being run for the first time and getting the parts and paint hot. There is no vapor emission from this stack, only heat. I will continue to search for a source. No violations.
0988724376 2014-04-19 Casing String Witnessed No Violations I was on location at 6:00pm to witness the cementing of the 13.375 surface casing. Schlumberger was on location and Wyoming Casing was running pipe. It was determined that the threads on the landing joint were damaged and would not properly thread. The rig circulated while waiting for a new landing joint to arrive from Nomac Rig #73. The joint arrived at 7:30pm and it was threaded on to the top of the string. Nomac #72 had to wash the last 20 of casing into the hole landing the shoe at 971 measured from the kelly bushing. Schlumberger rigged up and cemented the casing with 191 barrels of Class A cement weighing 15.6lbs/gal. The plug landed and 50 barrels of cement circulated to surface. 1434384166 2015-04-28 Production Wells No Violations I meet with the Federal EPA at this location. They performed an Air Quality Inspection of the well and tank battery. I performed a status check of the tank battery and well head. ID signs are posted. No violations were noted at this time.
1451921539 2015-12-28 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1977391278 2016-02-19 Preliminary Restoration No Violations I was on location for a status check. My inspection found the access road gate is open and the access road is in good repair. Identification signs are posted at the access road entrance, well and storage tanks. The pad is fenced and all man doors were locked. The #6H is in production. The well is equipped with a Christmas tree valve assembly. There is a steel dike around the storage tanks.

The equipment on the pad consists of the following: 6 Storage tanks 3 Sand separators 3 Gas processing units (GPU) 1 Vertical separator 1 Vapor recovery unit (VRU) 1 Heat treater 1 Thermal electric generator (TEG) 3 Production wells on the pad Site fenced and man doors locked
