Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
-320955085 2014-2-11 0:00: DD No Violations 3.00
-447088872 2020-4-2 0:00:0 PL No Violations 1.00
0262882049 2019-01-31 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 450 psi. The well was recently transferred to PennEnergy Resources LLC, and updated identification was posted. No violations were observed at the time of inspection.
0320955085 2014-02-11 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Rocky Mountain Drilling has drilled a 26 inch hole to a total depth of 100 feet and ran 100 feet of 20 inch casing (70 pounds per foot / inside diameter is 19.25 inches). Universal cemented the casing with 175 sacks of Type 1 cement with 2% CaCl and .125 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.2 gallons of mix water per sack. Universal pumped 37 barrels of cement slurry, 31 barrels of displacement, and there was 5 barrels of cement returns. My inspection revealed no violations at this time.
0327748040 2017-06-29 Production Wells No Violations I was on site with DOGRM Tom Hill, Bob Worstall, and Terry Whitehair, along with R.E Gas Development personnel to drilling an additional three wells (PN# 22553, 22732, 22733) on the pad. I performed a walk-around inspection of the pad and well while on site. The well is equipped with a plunger-lift assembly. I was unable to locate a clear surface-intermediate pressure gauge on the well. I spoke to R.E. Gas Development to have a gauge installed on the surface-intermediate annulus and labelled.
0623493795 2014-02-20 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 inch intermediate casing. I arrived on site and spoke with the security guard and he advised that cement trucks were not on site because the township road has been torn up and the trucks couldnt travel on the road. Rex Energy has a Road Use Agreement to come to the site off State Route 43 and Gallo Road ( Gallo Road is the road that has the issues). I went to the pad to talk with the Company Man ( Aaron Wilhite) and found that he was not in his office. I observed Summit Casings Truck was on site but no crews were with the truck. I also observed that the 9 5/8 inch casing was still on the pipe racks. While departing the site, I spoke with the security guard again and he advised me that the casing crew left sometime around midnight. NOTE: I tried to call the rig at 12:03am before leaving my house, because Aaron Wilhite had advised me earlier that they would be ready to cement at approximately 1:00am. I didnt get an answer when I called the rig, so I assumed Aaron was on the rig floor or rigging up cementers.
1393868827 2014-03-03 Drill / Deepen / Reopen No Violations I was on site with Inspector Jim Bucksar to conduct an inspection of the drill site. We walked the entire perimeter of the site and found that the filter socks and silt fence are intact and functioning properly. Our inspection of the rock channels revealed that the channels were intact and there was no erosion and sedimentation. We walked around the drilling rig and containment areas and found no spills on the containment or holes in the containment. Our inspection revealed no violations at this time. NOTE: We notified the Company Man and the Toolpusher that we were on site and were going to conduct an inspection.
1660737709 2019-8-27 0:00: PW No Violations 0.90
1756115251 2018-06-27 Production Wells No Violations I conducted a status check of the well and production pad. Seven producing wells are located on the pad (permit numbers (2528, 2530, 2498, 2554, 2486, 2553, 2529). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of zero psi. Identification was posted at the well and lease entrance. The production tanks and production equipment were surrounded by a lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. I tested the standing water for chlorides using a low-range QuanTab which concluded in a reading of less than 33 ppm. The access road was stable and drivable. Erosional controls were in place and appeared functioning.

Equipment on the pad consisted of the following: Fifteen 400 bbl production tanks; eight labelled as containing condensate and seven as containing produced water Seven gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower

2065986877 2017-06-15 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #6H well is in production and is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. A sound barrier wall was installed on the north side of the pad.

The equipment on the pad consists of the following: 7 producing wells

7 sand separators 7 gas processing units 1 vapor recovery unit 1 heat treater

No violations were noted at the time of inspection.

2070585789 2014-11-19 Production Wells No Violations On site with District Supervisor - Richard Anderson and Regional Supervisor - Tom Hill to conduct a status check on the production well number 6H. At the time of the inspection, the site was frozen and snow covered. The equipment on site related to the 6 well heads also contained 12 storage tanks, 6 sand separators, 6 gas processing units, 1 oil stabilizer, 1 main service line, and 6 gas recovery units. During the inspection, Unit #2 of the gas recovery unit had a leaking dump valve whose gas pressure fell below correct operating pressure and needed to be reset. Contacted the well tender to come out and reset the unit.
