0652101452 2017-11-01 Production Wells No Violations I conducted a status check of the well and pad. No changes have been made to the production equipment since my last inspection. The well is equipped with a plunger-lift unit and the surface-intermediate annular gauge displayed a reading of zero psi. The lease road was in good repair. No violations were observed at the time of inspection.

Express Energy Services weled 3 joints of 20 pipe and cemented the same back to the cellar floor. No violations were identified during this inspection.
1409159749 2014-08-25 Casing String Witnessed No Violations I was on location to witness the placement and cementing of the 20 conductor casing. Top Hole Drilling drilled a 26 borehole to 120 deep from ground level. Three joints of 20 casing ( 94 lbs. / foot ) were welded together as it was lowered into the bore hole. Windows were cut into the bottom of the casing five feet from the bottom to allow circulation of the cement slurry. Express Energy Services connected to the casing and pumped 48.34 barrels of cement slurry and displaced the slurry until 17 barrels of cement slurry was observed at the surface. Water temp. - 77, pH - 7.4 . Cement top was observed to have fallen downhole to 30 after one hour. The borehole was topped off with 1 cubic yard of grout. No violations were observed.
1417785272 2017-03-02 Preliminary Restoration No Violations

I was on location to conduct a preliminary restoration inspection. The #9H well is in production and consists of an open flow assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of twelve 400 bbl production tanks, four labeled as containing produced water, and eight labelled as containing condensate. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of: 12- 400 bbl production tanks, four labelled as containing produced water, and eight labelled as containing condensate 8 sand separators 8 gas processing units 1 low pressure vertical separator 1 oil stabilization unit 1 vapor recovery unit 1 horizontal separator, 1 compressor station Two bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

1920686987 2014-10-15 Casing String Witnessed No Violations I was on location to witness the placement and cementing of the 13.375 surface casing. Patterson Rig # 329 drilled a 17.5 borehole to 617 KB and placed 606.13 of 13.375 , 54.5 lbs. / foot casing in the hole. Five ( 5 ) bowspring centralizers, 13/375 float shoe and float collar were included in the casing string. Schlumberger cemented the casing back to surface with 598 sacks of cement. I observed approximately 37 barrels of cement returned back to the cellar. Once the plug was landed Schlumberger increased the pressure to 786 psi and held it there for 5 minutes. No pressure drop was observed. No violations were identified during this inspection.
1995285063 2014-08-21 Casing String Witnessed No Violations I was on this location to witness the placement and cementing of the 13.375 surface casing. Patterson Rig #329 drilled a 17.5 borehole to 635 KB and then ran 622.7 KB of 13.375 54.5 lbs. / foot casing. Schlumberger cement the casing to ground level with 535 sacks of Surfaceset cement. Water temp was 71 and pH was 7. I observed 27 barrels of cement back to surface. No violations were identified during this inspection.
