I was on location to conduct a preliminary restoration inspection. The #2H well is in production and consists of an open flow assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of twelve 400 bbl production tanks, four labeled as containing produced water, and eight labelled as containing condensate. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of: 12- 400 bbl production tanks, four labelled as containing produced water, and eight labelled as containing condensate 8 sand separators 8 gas processing units 1 low pressure vertical separator 1 oil stabilization unit 1 vapor recovery unit 1 horizontal separator, 1 compressor station Two bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

0699314054 2014-08-24 Casing String Witnessed No Violations I was on location to observe the placement and cementing og the 9.625 intermediate casing string. Patterson Rig #329 drilled a borehole to 2200 KB and ran 2188.6 of 9.625 , 40 lbs. / foot casing to 2188.6 KB. Schlumberger cemented the casing in place back to surface with782 sacks of Surfaceset cement. Water temp. - 72, pH - 7. I observed 36 barrels of cement back to surface. No violations were identified during this inspection
1510692105 2017-11-01 Production Wells No Violations I conducted a status check of the well and pad. No changes have been made to the production equipment since my last inspection. The well is equipped with a plunger-lift unit and the surface-intermediate annular gauge displayed a reading of zero psi. The lease road was in good repair. No violations were observed at the time of inspection.

1757252967 2015-12-22 Production Wells No Violations I was on site at the Leggett Pad with DOGRM inspector Robert Roush to investigate after being notified by the DOGRM Engineering Department that the fill slope on the east side of the Pad has been showing seepage. After monitoring the chloride levels in the seepage water from the east fill slope since August of 2015, the Engineering department found a rise in the chloride levels on 12/18/2015. While on site, I tested for chlorides with a low-range QuanTab from the outside of the fence on the northeast corner of the pad and just north of the culvert, and reading of 218 ppm was obtained. I also tested the standing water located south of the culvert and obtained a reading of 66 ppm, and 58 ppm in the standing water above the culvert. There had been heavy rains in the area on the morning that we tested. I collected a water sample from the seepage in the culvert. The coordinates from the culvert where the sample was taken are 40.45570, -81.18117. I inspected the tank battery on the pad. There appeared to be oil sheen on top of the standing water inside of the dike surrounding the production tanks. I tested the standing water inside of the tank battery for chlorides with a low-range QuanTab, and observed a reading of 421 ppm. I collected a water sample from the standing water inside of the dike. All samples were taken to the DOGRM laboratory on 12/22/2015 for analysis. I reviewed the site. Chesapeake personnel were on site working on equipment. No violations were found.
1820422998 2014-07-14 Drill / Deepen / Reopen No Violations Top Hole Drilling augured a 26 hole to a depth of 120. Allied Oil & Gas established circulation with fresh water and pumped 247 sacks of class A cement with 2% Calcium, .125 pps Floseal and displace to a depth of 110 with 39 barrels of fresh water. There was 11 barrels of cement returned to surface.
