1386167918 2013-12-03 Drill / Deepen / Reopen No Violations I went to the site to witness the cementing of the intermediate casing. The drilling contractor, Nomac Rig # 73 , drilled a 12.25 diameter hole to 1918. There were no issues while drilling the intermediate hole. 9.625 OD casing was set at 1909 KB. A total of 15 centralizers were installed on the intermediate casing string. O-Tex was the cementing contractor. There was good fluid circulation throughout the cementing operation. O-Tex adjusted the cement density, and pumped 151 bbls of cement slurry at 15.6 ppg. Cement returns to the surface were observed. An estimated 33 bbls of cement returned to the surface. The plug was displaced with 138 bbls of water. The plug landed and was pressured up on. The float held when the pressure was released. 1.0 bbls of water bled back.
1777014807 2015-11-09 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1919627945 2013-12-01 Drill / Deepen / Reopen No Violations I was notified about the cementing of the surface casing by Inspecor Mike Dinsmore. The drilling contractor, Nomac Rig # 73, drilled a 17.5 diameter hole to 590. There were no issues while drilling the surface hole. 13.375 OD casing was set at 582 KB. A total of 5 centralizers were installed on the surface casing string. O-Tex was the cementing contractor. I was informed that there was good fluid circulation throughout the cementing operation. O-Tex pumped 115 bbls of cement slurry at 15.6 ppg. An estimated 23 bbls of cement returned to the surface. The cement was displaced with 82 bbls of water.
