0256003270 2014-02-10 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Enviro Drill has drilled a 30 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch casing (78.67 pounds per foot / inside diameter 19.25). Allied cemented the casing with 433 sacks of Class A 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 91 barrels of cement slurry, 39.5 barrels of displacement, and there was 8 barrels of cement returns to surface. My inspection revealed no violations at this time. NOTE: Curry Vac trucks were almost 2 hours late and they couldnt start the job without them (suppose to be on site at 5:00pm and they didnt arrive until almost 7:00pm).
0344895229 2016-02-22 Production Wells No Violations I conducted a status check inspection of this Utica Shale well pad. My inspection found the lease road gate present and locked. The lease road and apron are in good repair. The pad is fenced and the entrance gate and all man doors on the perimeter of the pad were locked. The #10H well is in production and the gauge on the tree cap indicates a pressure of 214psi. The production wing valve indicates a pressure of 428psi and is choked back and producing at a pressure 207psi to the surface production equipment. The proper ID is posted at the entrance and at the wellhead and tank battery. The pad is equipped with video surveillance.

The equipment on the pad consists of the following:

6-400bbl storage tanks (3 water, 3 oil/condensate) 1 Producing well 1 Sand separator 1 GPU 1 Heated oil stabilizer unit 1 Thermo-electric desiccator with a vertical drip bottle 1 Self-contained, 350 gallon, methanol tank containing Assure HI-18FB hydrate inhibitor 1 Metal compressor shed housing a natural gas powered compressor 1 Low pressure separator tower 1 Combustor with inline pressure tank and air compressor

0756585542 2015-09-14 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1394413000 2014-03-08 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 74 has drilled a 17 1/2 inch hole to a total depth of 485 feet and ran 470 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter is 12.615 inches). They installed 6 bowspring centralizers on the casing string, on joints 1,2,3,6,9, and 11. O-TEX cemented the casing with 440 sacks of Class H 2% CaCl, 0.4% C-41P, and 1/4 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.26 gallons of mix water per sack of cement. They pumped 94 barrels of cement slurry, 67 barrels of displacement, and there was 17 barrels of cement returns to surface. They bumped the plug with 257 PSI and the pressured up to 797 PSI for the casing test. NOTE: They attempted to conduct the casing test 3 times and the test failed 3 times. They are going to conduct a casing test before they drill out the plug and float collar. My inspection revealed no violations at this time. NOTE: Schlumberger could not cover the surface casing cement job so they contatcted O-TEX to cover the job. O-TEX was suppose to be on site around 12:30am to 01:00am, but they didnt arrive on site until approximately 5:00am.
