0350242038 2014-01-30 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 73 has drilled a 17.5 inch hole to a total depth of 545 feet and ran 535 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter 12.615 inches). They installed 5 bowspring centralizers on the casing string on joints 1,3,6,9, and 12. O-TEX cemented the casing with 500 sacks of Class H (D901) with 2% CaCl, , and 1/4 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.20 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 106.86 barrels of cement, 77 barrels of displacement, and there was 25 barrels of cement returns to surface. They bumped the plug with 160 PSI and the bumped the pressure up to 820 PSI to conduct the casing test. Casing test was approved, floats held, and there was approximately 1/2 barrel of flowback when the pressure was released. My inspection revealed no violations at this time.
0522529581 2018-10-16 Production Wells No Violations The well is equipped with a plunger-lift assembly. No changes have been made to the pad production equipment since my last inspection. The surface/intermediate annular gauge displayed zero psi and the intermediate/production annular gauge displayed approximately 200 psi. No violations were observed at the time of inspection.
0906556493 2015-09-14 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1046653962 2013-12-20 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express has drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pounds per foot / inside diameter 19.124 inches). Allied cemented the casing with 238 sacks of Class A with 2% CaCl and 1/2 pound per sack flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 50 barrels of cement, pumped 39 barrels of displacement, and there was 7 barrels of cement returns to surface. My inspection revealed no violations at this time.
1789952259 2017-03-17 Production Wells No Violations I conducted a status check of the well and pad. The pad houses six producing wells, and the well is equipped with a plunger lift assembly. The tank battery is surrounded by a steel dike and consisted of eight 400 bbl production tanks, five labelled as containing condensate, and three labelled as containing produced water. Identification was posted at the access road, the well, and tank battery. Other equipment on the pad consisted of:
