0057366185 2018-10-16 Production Wells No Violations The well is equipped with a plunger-lift assembly. No changes have been made to the pad production equipment since my last inspection. The surface/intermediate and intermediate/production annular gauges displayed zero psi. No violations were observed at the time of inspection.
0493468391 2015-09-14 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0518630145 2014-02-26 Drill / Deepen / Reopen No Violations Kyle Tharp and I were on location to witness the cementing of the 9 5/8 inch intermediate casing. Our inspection revealed that Nomac Drilling Rig # 73 has drilled a 12 1/4 inch hole to a total depth of 1927 feet and ran 1917 feet of 9 5/8 inch intermediate casing (40 pounds per foot intermediate casing / inside diameter 8.835 inches). They installed 14 bowspring centralizers on the casing string on joints 1,4,7,10,13,16,19,22,25,28,31,34,37, and 40. O-TEX cemented the casing with 710 sacks of ClassH 3% Gypsum, 2% CaCl, 0.8 MC-500, 0.5% C-45, 0.4% C-41P. The cement had a density of 15.8 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.14 gallons of mix water per sack. O-TEX pumped 153 barrels of cement, 138.5 barrels of displacement, and there was 34 barrels of cement returns to surface. They bumped the plug with 933 PSI and conducted the casing test with 1552 PSI. The casing test was approved, floats held when pressure was released, and there was 1 barrel of water flowed back. Our inspection of the well site revealed no violations at this time.
0939188871 2017-11-16 Production Wells No Violations I conducted a status check of the well and pad. The pad houses six producing wells, and the well is equipped with a plunger lift assembly. The surface-

                                                                                                                                                                                                                         The access road was in good repair.  No violations were observed at the time of inspection.

1611811464 2013-12-20 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express has drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pounds per foot / inside diameter 19.124 inches). Allied cemented the casing with 238 sacks of Class A with 2% CaCl and 1/2 pound per sack flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 50 barrels of cement, pumped 39 barrels of displacement, and there was 14 barrels of cement returns to surface. My inspection revealed no violations at this time.
2086143098 2014-02-25 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 73 has drilled a 17 1/2 inch to a 540 feet and ran 530 feet of 13 3/8 inch casing (54.5 pounds per foot / inside diameter of 12.615 inches). 5 bowspring centralizers were ran on the casing string on joints 1,3,6,9, and 12. O-TEX cemented the 13 3/8 inch casing with 500 sacks of Class H with 2% CaCl, 0.4% C-41P, and 1/4 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 107 barrels of cement slurry, 75 barrels of displacement, and there was 20 barrels of cement returns to surface. They bumped the plug at 02:08am with 321 PSI and then bumped the pressure up to 807 PSI for the casing test. The casing test was approved, floats held, and there was approximately 1/4 barrel flowback when the pressure was released. My inspection revealed no violations at this time.
