0133896421 2014-06-18 Surface Facility Construction No Violations I was on location to meet with Electrical Safety Inspector, Larry Elasivich from the Department of Commerce so he could inspect and approve the power supply to the production equipment on location. Larry conducted the inspection with Speelman Electric and checked all grounding and connections at the main power panel for the pad. The equipment passed inspection and a sticker was placed at the main panel. He will contact AEP to let them know the power can be turned on. No violations.
0796524916 2018-02-22 Production Wells No Violations I conducted a status check of the pad with DOGRM District Supervisor Stephen Ochs. The surface-intermediate and intermediate-production annular gauges both displayed a reading of zero psi. No violations were observed at the time of inspection.
0904807140 2013-12-01 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Sidewinder Rig #63. Sidewinder drilled an 8.5 hole to 6,200 and were currently tripping into the hole with drill string after making repairs to the tophead drive. All equipment is on containment and drilling fluids are contained in steel tanks. No violations.
0961225222 2016-01-27 Work Over Reconditioning No Violations I was on the Grunder Pad to review the status of the shut - in wells due to the drilling of the Grunder # 2H. A Baker Hughes wireline unit was over the hole trying to remove the kill plug. Larry Boone ( Rex co-man ) said that they were having trouble removing the kill plug, possibly due to hydrates ( methane solids ) built upon the top of the kill plug. Due to working conditions an inspection of the annular pressures could not be performed. No violations.
1139725296 2013-11-25 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Sidewinder Rig #63. The rig is currently drilling the 12.25 intermediate hole. All equipment is on containment. No violations.
1177753145 2017-06-30 Production Wells No Violations I was on site with DOGRM Tom Hill, Bob Worstall, and Terry Whitehair, along with R.E Gas Development personnel to drilling an additional three wells (PN# 22553, 22732, 22733) on the pad. I performed a walk-around inspection while on site. The well is equipped with a plunger-lift assembly I was unable to locate a clear surface-intermediate pressure gauge on the well. I spoke to R.E. Gas Development to have a gauge installed on the surface-intermediate annulus and labelled.

1419061193 2017-06-15 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #9H well is in production and is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. A sound barrier wall was installed on the north side of the pad.

The equipment on the pad consists of the following: 7 producing wells

7 sand separators 7 gas processing units 1 vapor recovery unit 1 heat treater

No violations were noted at the time of inspection.

1472694280 2018-06-27 Production Wells No Violations I conducted a status check of the well and production pad. Seven producing wells are located on the pad (permit numbers (2528, 2530, 2498, 2554, 2486, 2553, 2529). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of ten psi. Identification was posted at the well and lease entrance. The production tanks and production equipment were surrounded by a lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. I tested the standing water for chlorides using a low-range QuanTab which concluded in a reading of less than 33 ppm. The access road was stable and drivable. Erosional controls were in place and appeared functioning.

Equipment on the pad consisted of the following: Fifteen 400 bbl production tanks; eight labelled as containing condensate and seven as containing produced water Seven gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower

1649594624 2019-01-31 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed zero psi. The well was recently transferred to PennEnergy Resources LLC, and updated identification was posted. No violations were observed at the time of inspection.
1904786073 2013-11-26 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Sidewinder Rig #63 drilled a 12.25 hole to a depth of 2033 (KB) and ran 47 joints of 9.625, J-55, intermediate casing to a depth of 2030 (KB). The casing landed 4 above the rig floor and Universal rigged up the cement head. Universal established circulation and conditioned the borehole with 5 barrels of calcium, 5 barrels of Flostop, and 50 barrels of gel which were each followed by 5 barrel water spacers. Universal then mixed and pumped 142 barrels of cement slurry, dropped the plug, and displaced the plug to the float collar with 152 barrels of water. The plug landed at 822psi and the pressure was increased to 1400psi and held for 5 minutes. The pressure was released and the floats were holding. 28 barrels of good cement returned and held at the surface. See casing ticket for more details on this job. No violations.
