0407557755 2015-11-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1295943207 2016-03-11 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was locked. The #10H well is in production and the well is equipped with a plunger lift assembly. The pad is surrounded by a fence and all man doors were locked. The production tanks are surrounded by a steel dike. There was approximately six inches of standing water contained inside of the dike. I tested the standing water for chlorides using low-range QuanTabs which resulted in a reading of 216 ppm. The equipment on the pad consists of the following:

6- 400 bbl production tanks, three labelled as containing produced water and three labelled as containing condensate. 1 Low pressure vertical separator located inside of the dike. 7 sand separators 7 gas processing units 1 vapor recovery unit 1 oil stabilization unit 3 line heaters

1336667262 2018-08-29 Production Wells No Violations I conducted a status check of the well and production facility. The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate production gauge displayed a reading of 20 psi. I performed a walk around inspection of the pad and no violations were observed at the time of inspection.
1366438403 2018-11-06 Production Wells No Violations The well was equipped with an open-flow assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production gauge displayed 10 psi. No violations were observed on the production pad at the time of inspection.
1522680089 2018-04-02 Production Wells No Violations 22526 10H:

I conducted a status check of the pad. Equipment changes since my last inspection include removal of two line heaters and the oil stabilization unit. Equipment on the pad now consisted of :

- 400 bbl production tanks, three labelled as containing produced water and three labelled as containing condensate.





The surface/intermediate annulus displayed a reading of zero psi, and the intermediate/production displayed zero psi.  No violations were observed at the time of inspection
2083962972 2013-12-10 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express had drilled a 26 inch hole to a total depth of 110 feet and ran 110 feet of 20 inch conductor casing (94 pounds per foot casing / inside diameter 19.124 inches). Allied cemented the conductor casing with 210 sacks of Class A cement with 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 44 barrels of cement, 36 barrels of displacement, and there was 4 barrels of cement returns to surface. My inspection revealed no violations at this time.
