0299718381 2015-01-13 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 6, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found.

0448755650 2017-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #8H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 3 producing wells 6-400 bbl production tanks; three labelled as containing produced water and three labelled as containing condensate. 1 Low pressure vertical separator 3 sand separators

3 gas processing units
1 vapor recovery unit
I heat treater

1 compressor station No violations were found during my inspection.

0515171527 2015-07-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0559928613 2013-10-23 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Nomac Drilling Rig # 72 has drilled a 12 1/4 inch hole to a total depth of 1900 feet and ran 1887 feet of 9 5/8 inch casing (40 pound per foot / inside diameter is 8.835) into the wellbore. They installed 16 bowspring centralizers on the casing string (on joints 1,2,3,6,9,12,15,18,21,24,27,30,33,36,39, and 42). Schlumberger cemented the casing with 690 sacks of Class A (901) 1% D154, 0.4% D400, 0.4% D202, 0.2% D046, 1% S001, and .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.245 gallons of mix water per sack. Schlumberger pumped 148 barrels of cement slurry, 135.5 barrels of displacement , and there were 28 barrels of cement returns to surface. My inspection revealed no violations at this time.
0693580837 2013-10-11 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express had drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch casing into the wellbore. Allied cemented the casing with 250 sacks of Class A 2% CaCl and 1/2 pound per sack flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 53 barrels of cement, 39 barrels of displacement, and there was 7 barrels barrels of cement returns to surface. My inspection revealed no violations at this time
1385338750 2013-10-21 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Drilling Rig # 72 has drilled a 17.5 inch hole to a total depth of 525 feet and ran 513 feet of 13 3/8 inch casing (54.5 pound per inch/ inside diameter 12.615 inches) into the wellbore. They installed 6 bowspring centralizers on the casing string (on joints 1,2,3,6,8, and 11). Schlumberger cemented the casing with 470 sacks of Class A (D901) cement with 1% D154, 0.4% D400, 0.4% D202, 0.2% D046, 1.5% S001, and .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.24 gallons of mix water per sack. Schlumberger pumped 101.2 barrels of cement slurry, 73 barrels of displacement, and there were 21 barrels of cement returns to surface. My inspection revealed no violations at this time.
1579409262 2018-02-26 Production Wells No Violations I conducted a status check of the pad. No changes were made to the operating equipment since my last inspection. The well is equipped with a plunger lift unit and the surface-intermediate annular pressure gauge displayed a reading of zero psi. No violations were observed at the time of inspection.
