0120902650 2018-02-26 Production Wells No Violations I conducted a status check of the pad. No changes were made to the operating equipment since my last inspection. The well is equipped with a plunger lift unit and the surface-intermediate annular pressure gauge displayed a reading of zero psi. No violations were observed at the time of inspection.
0812706194 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was open at the time of inspection. The #8H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence. The man doors were unlocked, but Chesapeake personnel were on site at the time of inspection.

The equipment on the pad consists of the following: 8 -400 bbl production tanks, three labelled as containing produced water, and five labelled as containing condensate 1 low-pressure vertical separator 7 Producing wells 7 Sand separators 7 Gas Processing Units 1 Heat Treater 1 Vapor Recovery Unit

1149442269 2015-09-14 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1315645500 2013-11-22 Drill / Deepen / Reopen No Violations I was on location with inspector Kevin Perry to inspect drilling operations of Patterson Rig #261. They are currently attempting to retrieve the bottom of the air hammer that sheared off during drilling operations. All equipment is on containment. No violations.

Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. 1744389332 2017-03-13 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #7H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 7 producing wells

1 Low pressure vertical separator 7 sand separators 7 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station

No violations were found during my inspection.

1986282060 2013-12-02 Drill / Deepen / Reopen No Violations I was notified by the company man for Patterson Rig #261 that they had the drill pipe stuck in the hole at 6138. After unsuccesful attempts in trying to retrieve the drill pipe. There was 189 of drill pipe left in the hole from 5949 to 6138. O-Tex was the cement contractor used to set a cement plug on top of the drill pipe. They set a 793 cement plug from 5127 to 5920. There was 59 BBLs of Class A cement that mud scaled at 17.5 ppg used during the job.
