0519550726 2017-10-30 Production Wells No Violations The well is equipped with a plunger-lift assembly. No changes have been made to the pad production equipment since my last inspection. The surface/intermediate annular gauge displayed zero psi and the intermediate/production annular gauge displayed approximately 300 psi. No violations were observed at the time of inspection.
1381118801 2013-10-06 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express Energy Services has drilled a 26 inch hole to a total depth of 120 feet and ran 20 inch conductor casing to a depth of 120 feet. Allied cemented the casing with 236 sacks of ClassA cement (2% CaCl and 1/2 pound per sack floseal). The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.2 gallons of mix water per sack. Allied pumped 53 barrels of cement slurry, pumped 39 barrels of freshwater displacement, and there was 13 barrels of good cement returns to surface. My inspection revealed no violations at this time.
1383242329 2013-10-31 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Nomac Drilling Rig # 73 has drilled a 17.5 inch hole to a total depth of 718 feet and ran 708 feet of 13 3/8 inch casing (54.5 pound per foot / inside diameter 12.615 inches) into the wellbore. They installed six bowspring centralizers on the casing string (on joints 1,2,5,8,11,and 14). OTEX cemented the casing with 640 sacks of Class H 2% CaCl, 0.4% C-41P, and 1/4 pound per sack flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. OTEX pumped 136.78 barrels of cement, pumped 102 barrels of freshwater displacement, and there were 23 barrels of good cement returns. The cement returns weighed 15.6 pounds per gallon. They landed the plug with 485 PSI and bumped the pressure up to 990 PSI for the casing test. Casing test approved and approximately 1/2 barrel of flowback when the pressure was released. My inspection revealed no violations at this time.
1506007903 2017-10-30 Production Wells No Violations I conducted a status check of this well. Identification was posted at the access entrance, the wells, and tank battery. The well is in production and was equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: -400 bbl production tanks, three labelled as containing condensate, and three labelled as containing produced water -pressure vertical separator



The access road was in good repair. I did not locate a surface-intermediate annular gauge. I contacted Chesapeake to have a gauge installed.

1830502798 2015-07-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

No changes have been made to the production equipment since my last inspection. Erosional controls appeared to be in place and functional. No violations were observed at the time of inspection.
