0083019006 2018-06-21 Production Wells No Violations The well is equipped with a plunger-lift unit. The surface-intermediate annular pressure gauge displayed a reading of zero psi, and the intermediate-production annular gauge displayed 250 psi. No changes to the pad since my last inspection. No violations were observed.
0357091217 2017-06-19 Preliminary Restoration No Violations

I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, three labeled as containing condensate, and five labelled as containing produced water, and one low-pressure vertical separator. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of: 8 -400 bbl tanks, three labelled as containing fresh water and five labelled as containing condensate 3 producing wells 3 sand separators 3 gas processing units 1 vapor recovery unit 1 heat treater

Jersey barriers are installed around the production equipment.  The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire.  The  man doors were locked.   Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road was stable in in good repair.
0564622049 2013-08-25 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing (94 pound per foot and inside diameter 19.124 inches). My inspection revealed that Butchs Rathole had drilled a 30 inch hole to a depth of 120 feet and ran 120 feet of 20 inch conductor casing into the well bore. Allied cemented the casing with 462 sacks of Class A ( 2% CaCl and 1/2 pound per sack flake) with a 1.18 yield. They pumped 97 barrels (15.6 pounds per gallon density)of cement slurry, displaced with 39 barrels of freshwater, and got 22 barrels of cement returns to surface. No violations at this time. NOTE: Spudded well on 08/24/2013.
0738659328 2013-09-18 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. My inspection revealed that Nomac Rig # 73 has drilled a 12.25 hole to a total depth of 1514. Franks Casing Crew ran 1504 of 9 5/8 casing with 11 bowspring centralizers installed on the casing string (on joints 1,4,7,10,13,16,19,22,25,28, and 31). O-TEX cemented the casing with a lead cement of 90 sacks (19.87 barrels) of ClassH 10% NaCl, 0.4% C-41P, 0.1% C-35, and 1/4 pound per sack cello-flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.24 cubic feet per sack, and they used 5.48 gallons of mix water per sack. They pumped a tail cement 460 sacks (99.13 barrels) of Class H 3% CaCl, 0.4% C-41P, 0.1% C-35, and 1/4 pound per sack cello-flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.23 gallons of mix water per sack. O-TEX pumped a total of 119 barrels of cement slurry, pumped 107 barrels of displacement, and there was 21 barrels of good cement returns to surface. They landed the plug at 2:45am with 810 PSI and the bumped the pressure up to 1315 PSI and conducted the casing test for 5 minutes. Casing test was approved at this time so they released the pressure. The floats held and the flowback was approximately 1/2 barrel. No violations at this time.
1379336330 2013-09-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 casing. My inspection revealed that Nomac Rig # 73 has drilled a 17.5 inch hole to a total depth of 835 feet and ran 826 feet of 13.375 inch ( 54.5 pounds per foot / inside diameter of 12.615 inches) with seven bowspring centralizers installed on the casing string (on joints 1,3,6,9,12,15,18). O-TEX cemented the casing with 790 sacks of ClassH 2% CaCl, 1/4 pound per sack cello-flake, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 168.83 barrels of cement slurry, pumped 122 barrels of freshwater displacement, and there was 46 barrels of cement returns to surface. Landed the plug at 9:46am with 400 PSI and then conducted the casing test at 950 PSI for 5 minutes. Casing test approved and floats held when the pressure was bled off (1/2 barrel flowed back).

I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
