0180155295 2013-09-22 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole has set a 30 inch sleeve to a depth of 12 feet below the base of the tin horn. They drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pound per foot with an inside diameter of 19.124 inches). O-TEX cemented the casing with 250 sacks of ClassH cement 2% CaCl, 1/4 pound per sack cello-flake, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, yield of 1.20 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 53.43 barrels of cement slurry, pumped 39 barrels of displacement, and there was 10 barrels of cement returns to surface. My inspection revealed no violations at this time.
1272224210 2015-10-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1389667089 2013-12-31 Drill / Deepen / Reopen No Violations I was on site to check the status of Nomac Drilling Rig # 77. My inspection revealed that Nomac Rig # 77 was drilling the lateral section of the well at 12,100 feet. My inspection revealed no violations at this time.

1889111933 2017-03-07 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #10H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 4 producing wells

1 Low pressure vertical separator located inside of dike 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station There was approximately four inches of standing water inside of the dike. I tested the standing water with a low-range QuanTab which concluded in a reading of under 30 ppm.
