0330410557 2013-06-24 Drill / Deepen / Reopen No Violations I was onsite to check the status of the well pad completion. Nomac 72 will be coming to this location following the completion of the Putnam wells. Currently the conductor is drilled for this well and is awaiting the rig. Erosion controls have been put into place but the vegetation has not yet started to grow. Slag piles are still on the pad but will be put into place today.
1465988095 2017-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The pad is equipped with nine producing wells. The well was equipped with a plunger-lift assembly. The surface--lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

One low-pressure vertical separator Five line heaters Nine sand separators Nine gas processing units One combustor unit One heat treater Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration can be passed.

1813882308 2013-07-02 Casing String Witnessed No Violations I was onsite with inspector Brett Farnsworth to witness the cementing of the 6H and 3H conductors for this location. Currently the pad is only partially completed. The location has been graded and had partial amounts of erosion control and drainage installed. The conductors have been drilled before any slag or gravel has been laid on the pad. This conductor was drilled to 120 and would be cemented to surface. There was a delay in the job as vacuum trucks were late on arrival. The cement job was pumped as planned and in total around 9 barrels of cement were returned to surface and CaCl was added to quick set the cement. There are no violations at this time.
