0616450887 2013-08-24 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole drilled a 26 inch hole to a depth of 18 feet and ran a 26 inch sleeve to a depth of 18 feet. Precision then drilled a 24 inch hole to a depth of 120 feet and ran 120 feet of 20 inch conductor casing. Allied cemented the casing with 230 sacks of Class A 2% CaCl and 1/2 pound per sack cello flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and 5.2 gallons of freshwater per sack of cement. Allied pumped 48 barrels of cement slurry, 39 barrels of freshwater displacement, and there was 18 barrels of cement returns to surface. My inspection revealed no violations at this time.
1129675301 2015-01-20 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found.

1203056489 2017-10-16 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. Three producing wells were located on the pad #(22429, 22430, 22433). The well is equipped with a plunger lift unit. There were also three wells on site that were drilled to conductor depth and were capped #(22431, 22432, 22434).

The equipment on the pad consists of the following: 6 - 400 bbl production tanks, three labelled as containing produced water, and three labelled as containing condensate. The tanks were surrounded by a steel dike. 1 Low pressure vertical separator 6 Sand separators 6 Gas processing units 1 Heat Treater 1 Combustor unit.

The access road was stable and drivable. No violations were noted at the time of inspection.

1594150333 2015-07-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
