0146895105 2013-07-29 Drill / Deepen / Reopen No Violations I was onsite to witness the drilling of the 10H and 12H conductors. Normally Express will drill the cellars and then drill the conductor and mouse holes. In an effort to save time they are going to drill the conductors and then dig our of the cellars. They expect to have the wells drilled today and cement the conductors the following day.
0217260400 2013-08-13 Administrative Inspection No Violations I was contacted by Nomac 72 informing me that they should be cementing the intermediate casing sometime in the early mourning. They have currently TD their section and are going to pull out of the hole to run a cement bond log over their surface casing.
0490241873 2013-08-14 Drill / Deepen / Reopen No Violations I was onsite to check the status of Nomac 72. They are currently waiting on cement for 8 hours. Currently they are cutting and slipping the drill line. Following this they will trip into the hole and blow the hole clean in order to start drilling their 8.75 vertical section.
0503010572 2013-07-30 Casing String Witnessed No Violations I was on location to witness the cementing of the 12H well conductor set to 120 feet. The well was pumped as planned and in total 8 barrels of cement were returned to surface. They are going to sit on the wells for 4 hours before drilling the mouse holes for the wells. One they are done they will walk the rig over the 10H well.
0739130372 2013-06-20 Not Drilled No Violations I was inspecting this location to determine how much longer construction has before the location can be completed. Nomac 72 is going to be moving to this location when they completed the final well on the Putnam well pad. Currently an auger rig is on location drilling conductors for the location. I was not able to access the pad due to a very large number of slag trucks waiting to access the pad. Erosion controls have been installed around the location but growth has not been established yet. It appears the location is still several days away from being completed. There are no violations at this time.
1240784297 2013-06-24 Not Drilled No Violations I was onsite to check the status of the well pad completion. Nomac 72 will be coming to this location following the completion of the Putnam wells. Currently no conductor has been drilled for this well. This well is permitted as a stratograph well and it is likely that Chesapeake will not drill it. They will likely repermit the well for strictly a horizontal well.
1308476950 2015-10-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1376266819 2013-08-11 Drill / Deepen / Reopen No Violations I was contacted by Nomac 72 informing me that they are going to be spudding the well tonight and expect to run can cement casing tomorrow.
1376594219 2013-08-12 Casing String Witnessed No Violations I was onsite to witness the cementing of the 13 3/8 surface casing set to 582 feet. The job was pumped as planned and 114.5 barrels of cement weighing 15.6ppg was pumped down hole. In total 32 barrels of good cement were returned to surface. They are going to drill their intermediate hole o roughly 1900 feet after they wait on cement.
1425406829 2013-08-21 Drill / Deepen / Reopen No Violations I was on location to check the status of the drilling of this well. Currently the rig has drilled the entire well and is tripping out with 1400 of drill pipe still to lay down. They expect to be cementing the 5.5 prioduction casing tommorrow afternoon.

1739192673 2017-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #12H well is in production and is equipped with an open flow assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 4 producing wells

1 Low pressure vertical separator located inside of dike 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station

No violations were found during my inspection.

1767972906 2013-08-22 Casing String Witnessed No Violations I was onsite to witness the cementing of the 5.5 production casing for this well. The job was pumped as planned and mudpush was returned to surface as per the engineered plan. The pug was bumped at 2680psi and was held at 3480 psi. The floats did not hold and 4.5 barrels were bleed back to the truck. Another 1.5 barrels were pumped to hold the pressure to 3600 psi and again the floats did not hold. They are going to shut in the well and weight on cement for 12 hours before they attempt to bleed back the floats. Pressure will be monitored as to not overpressure the cement and equipment.
2097752832 2013-07-29 Administrative Inspection No Violations I was contacted by Chesapeake informing me that they are going to begin drilling the 10H and 12H conductors on this location. They stated that Express Energy is going to come in and drill both conductors to 120 feet and cement them the following day.
