0043558003 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was not locked. The #1H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence. The man gates were not locked, but third party personnel were on site at the time of inspection.

The equipment on the pad consists of the following: 8 - 400 bbl production tanks, three labelled as containing produced water and five labelled as containing condensate, with one low-pressure vertical separator located inside of the dike 6 Producing wells 6 Sand separators 6 Gas Processing Units 1 Heat Treater 1 Vapor Recovery Unit

0822762587 2015-10-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1053490075 2018-11-01 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 430 psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection.
1376828165 2013-08-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing (94 pound per foot / inside diameter 19.124). My inspection revealed that Express Energy Services has drilled a 26 hole to a total depth of 120. They ran 120 of 20 casing with 5 feet sticking up above ground. There is 115 of casing in the well bore from ground level. OTEX cemented the casing with 260 sacks of Class H (2% CaCl, 1/2 pound per sack cello-flake, and 0.4% C41-P (defoamer). The cement density was 15.6 pounds per gallon, yield was 1.2 cubic feet per sack, and they used 5.25 gallons per sack mix water. There was 55.57 barrels of cement slurry, 39 barrels of fresh water displacement, and 19 barrels of cement returns to surface. NOTE: They were worried about loss circulation problems so they pumped a 15 barrel gel slug ahead of the cement (200 pounds of gel and 40 pounds of Baroseal). No violations at this time.
