0485989254 2014-08-07 Production Wells No Violations Follow up to Complaint #4581 concerning noticeable vibrations at a home located at 5464 Steubenville Road SE (SR 43) and related to the production of this well and PN#22447. I was on location with supervisor Tom Hill to investigate the complaint. My inspection found both wells in production and equipped with a methanol treatment system. Both wells produced through a separate heated stabilizer, sand separator, and heated gas production unit (GPU) into 6-400 barrel tanks. The top of the tanks were plumbed to a large vapor combustor unit that can be a source of low frequency energy pulses and vibration during initial production of a Utica well. I found the combustor operating quietly with very little noise or vibration emanating through the louvers on the bottom of the unit. A visit to the complainants property (0.1 miles NE of the combustor) found that the property had been sold. Tom spoke with the new landowner and she told him that she did not have any issues with noise or vibrations at her home. I did not notice or observe any noise or vibrations emanating from the production operations while at the property. This complaint is resolved. No violations.
1186622476 2017-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The pad is equipped with nine producing wells. The well was equipped with a plunger-lift assembly. The surface--lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

One low-pressure vertical separator Five line heaters Nine sand separators Nine gas processing units One combustor unit One heat treater Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration can be passed.

1639142649 2013-07-02 Casing String Witnessed No Violations I was onsite with inspector Brett Farnsworth to witness the cementing of the 6H and 3H conductors for this location. Currently the pad is only partially completed. The location has been graded and had partial amounts of erosion control and drainage installed. The conductors have been drilled before any slag or gravel has been laid on the pad. This conductor was drilled and cemented to 120 the night before but needed to be grouted from the top today. CaCl was added to the cement to enable to it quick set as well. There are no violations at this time.
