I conducted a status check of the well and production pad. Three wells are located on the pad (permit numbers 2412, 2411, 2131). This well is equipped with a plunger-lift assemly. Proper identification was posted at the well and lease entrance. The production tanks, gas processing units, and vertical separators were surrounded by a steel dike. The access road was stable and drivable. DOGRM inspector Michael Dinsmore observed slumping east of the well pad during an inspection in 2014. I located the historic slumping referenced in the inspection but found no indication of recent movement at the time of inspection and the area appeared stable. Equipment on the pad consisted of the following: Nine 400 bbl production tanks; six labelled as containing condensate and three as containing produced water One low-pressure vertical separator tower Three gas processing units Three vertical separators One combustor unit.

I did not locate a surface-intermediate annular pressure gauge. I contacted operator who stated they in the process of acquiring gauges from the supplier and an annular gauge will be installed as soon as possible.

1380091804 2013-09-25 Drill / Deepen / Reopen No Violations Kyle Tharp and I were on site to witness the cementing of the 9 5/8 inch intermediate casing. Upon arrival to the site we witnessed Sidewinder Rig # 63 was still running the intermediate casing. They had approximately 15 joints of casing left to run into the well bore. They installed 13 centralizers on the casing string; on joints 1,3,7,11,15,19,23,27,31,35,39,43,46. Universal cemented the casing with 615 sacks of Gas Block cement 2% CaCl and 0.3 CR-3. The cement had a density of 15.7 pounds per gallon, a yield of 1.29 cubic feet per sack, and they used 5.54 gallons of mix water per sack. Universal pumped 141 barrels of cement slurry, 148 barrels of freshwater displacement, and they had 20 barrels of cement returns to surface. Universal bumped the plug at 06:04 am with 991 PSI and then conducted the casing test at 1533 PSI for 5 minutes. The casing test was approved at this time. Released the pressure and got 1 barrel of water flowback and the floats held. Our inspection revealed no violations at this time.
