0201826438 2016-02-22 Production Wells No Violations I was on location for a status check. My inspection found the access road gate is locked and the access road is in good repair. Identification signs are posted at the access road entrance, well and storage tanks. The pad is fenced and all man doors were locked. The #10H is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the storage tanks.

The equipment on the pad consists of the following: 6 Storage tanks 1 Sand separator 1 Gas processing units (GPU) 1 Vertical separator 1 Vapor recovery unit (VRU) 1 Thermal electric generator (TEG) 1 Production wells on the pad Site fenced and man doors locked

0661942699 2017-07-13 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #10H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 1 producing well

1 Low pressure vertical separator 1 sand separator 1 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station No violations were noted at the time of inspection.

0744474861 2018-04-11 Production Wells No Violations I conducted a status check of the well and pad. This well is equipped with a plunger-lift assembly, and is the only producing well on the pad. The S-I annular pressure gauge displayed a reading of zero psi, and the I-P annular gauge displayed a reading of 288 psi. No violations were observed at the time of inspection.
0839115491 2013-08-03 Administrative Inspection No Violations Nomac 73 contacted me and informed me that they are expecting to be setting intermediate casing sometime this mourning. They are currently rigging up the casing crew on the rig.
1051754163 2015-12-04 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1179238137 2013-08-01 Administrative Inspection No Violations I was contacted Nomac 73 letting me know that they have TD the well and are going to be spending several hours of open hole logging before they begin to run casing and prepare to cement the surface casing.
1375712535 2013-08-01 Casing String Witnessed No Violations I was on location to witness the cementing of the surface casing. I was able to cover the second half of the job due to covering another surface casing job in the area. The job was pumped as planned and cement weighing in at 14.8ppg was returned to surface. There are no violations at this time.
1493414440 2013-07-31 Administrative Inspection No Violations I was contacted by Nomac 73 at this location to see if I could cover the surface cement job of this well. Currently Brett Farnsworth and Rick Anderson are on vacation or busy with other jobs. I am going to cover the surface cement job and they are going to call me the following day to let me know when the estimated cement job will occur.
