0179414411 2015-10-20 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0277623258 2017-07-12 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #3H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 2 producing wells

2 Low pressure vertical separators 2 sand separators 1 gas processing unit 1 vapor recovery unit 1 heat treater

The access road was stable. No violations were noted at the time of inspection.

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The Mudpush spacer was mudscaled at 12.8 pounds per gallon (ppg). Schlumberger pumped 295 bbls of lead cement at 13.5 ppg, and 314 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 298.5 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 11:39am on 06/11/2013. The plug landing pressure was 2488 psi, and it was raised to 2998 psi, and held for 5 minutes. Pressure fell slightly to 2991 psi. The float held when the pressure was released. 3.0 bbls of water bled back. A total of 14 bbls of the 60 bbl Mudpush spacer returned to surface.
