0364372626 2015-11-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0446869546 2018-12-18 Production Wells No Violations I was on site in response to complaint #6031. I received the complaint on 12/13/2018 from caller who wished to remain anonymous and was concerned of dozer work at the Shaw Properties freshwater impoundment causing stability issues of the pad slope. This freshwater impoundment is located west of the Shaw Properties producing pad. DOGRM Engineer Paul Logan was on site on 12/14/2018 to access the location. He spoke to a contractor who was on site doing dozer work. The contractor was working for the landowner and was removing soil to fill in a wet area located at the base of the west slope of the pad. The contractor stated that no more than two feet of soil has been removed. The DOGRM engineer observed no stability issues on 12/14/2018. I located the disturbed area on 12/20/2018, and I observed no erosional and stability issues or concerns. The complaint can be closed at this time.
0501332021 2017-11-16 Production Wells No Violations I conducted a status check of the well and pad, which is equipped with seven producing wells. The well is equipped with a plunger-lift assembly and the surface-

Seven sand separators Seven gas processing units One combustor unit One low-pressure vertical separator One heat treater One stabilization unit

The access road was in good repair. No violations were observed at the time of inspection.

1379608410 2013-09-19 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Drilling Rig # 329 has drilled a 17.5 inch hole to a total depth of 790 feet. Wyoming Casing Crew ran 775 feet of 13 3/8 inch casing and installed seven bowspring centralizers on the casing string (on joints 1,2,3,6,9,12,15). O-TEX cemented the casing with 710 sacks of ClassH 2% CaCl, 0.4% C-41P, and 1/4 pound per sack cello-flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. They pumped 151.74 barrels of cement slurry, pumped 113 barrels of freshwater displacement, and got 40 barrels of cement returns to surface. O-TEX landed the plug at 01:12 pm with 350 PSI and pressured up to 900 PSI for the casing test. O-TEX held the pressure for 5 minutes and the casing test was approved. Flowed back 1/2 barrel and the floats held.
2128183230 2018-12-18 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 40 psi. I performed a walk-around inspection of the pad and no violations were observed at the well or production pad at the time of inspection.
446869546 2018-12-18 0:00 PW No Violations 0.70
