0857106826 2013-10-31 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Patterson Drilling Rig # 329 has drilled a 12.25 inch hole to a total depth of 2113 feet and ran 2098 feet of 9.625 inch casing (40 pound per foot / 8.835 inches inside diameter) into the wellbore. They installed 17 bowspring centralizers on the casing string (on joints 1,2,3,6,9,12,15,18,21,24,27,30,33,36,39,42,and 45). O-TEX cemented the casing with a lead cement of 140 sacks of Class H 10% NaCl, 0.4% C-41P, 0.1% C-35, and 1/4 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.24 cubic feet per sack, and they used 5.48 gallons of mix water per sack. They also pumped a tail cement of 610 sacks of Class H 3% CaCl, 0.4% C-41P, 0.1% C-35, and 1/4 pound per sack of celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.25 gallons of mix water per sack. They pumped 30.92 barrels of lead cement slurry and 131.45 barrels of tail cement slurry (toatal cement slurry of 162.37 barrels). O-TEX pumped xxxx barrels of freshwater displacement and there were xx barrels of good cement returns to surface. They landed the plug at xxxx with xxxx PSI and bumped the pressure up to xxx PSI for the casing test.
1383245417 2013-10-29 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Patterson Rig # 329 has drilled a 17.5 inch hole to a total depth of 827 feet and ran 813 feet of 13.375 inch casing (54.5 pound per foot / 12.615 inches inside diameter) into the wellbore. They installed 8 bowspring centralizers on the casing string (on joints 1,2,3,6,9,12,15,and 18). O-TEX cemented the casing with 720 sacks of ClassH 2% CaCl, 0.4% C-41P, and 1//4 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 153.88 barrels of cement slurry, pumped 120 barrels of freshwater displacement, and there were 32 barrels of good cement returns to surface. They landed the plug with 360 PSI and bumped it up to 860 PSI for the casing test. The casing test was approved. They released the pressure and there was 1/2 barrel of flowback and the floats held. My inspection revealed no violations at this time.
1585773541 2015-11-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1844872711 2017-11-16 Production Wells No Violations I conducted a status check of the well and pad, which is equipped with seven producing wells. The well is equipped with a plunger-lift assembly and the surface-

The access road was in good repair. No violations were observed at the time of inspection.

1853892709 2018-12-18 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 300 psi. I performed a walk-around inspection of the pad and no violations were observed at the well or production pad at the time of inspection.
1856064669 2018-12-18 Production Wells No Violations I was on site in response to complaint #6031. I received the complaint on 12/13/2018 from caller who wished to remain anonymous and was concerned of dozer work at the Shaw Properties freshwater impoundment causing stability issues of the pad slope. This freshwater impoundment is located west of the Shaw Properties producing pad. DOGRM Engineer Paul Logan was on site on 12/14/2018 to access the location. He spoke to a contractor who was on site doing dozer work. The contractor was working for the landowner and was removing soil to fill in a wet area located at the base of the west slope of the pad. The contractor stated that no more than two feet of soil has been removed. The DOGRM engineer observed no stability issues on 12/14/2018. I located the disturbed area on 12/20/2018, and I observed no erosional and stability issues or concerns. The complaint can be closed at this time.
