-based mud used to drill the hole and condition the hole, respectively, a lead cement followed by a tail cement were pumped. The lead cement was mudscaled at 14.4 pounds per gallon (ppg), and the density adjusted. Halliburton pumped 157 bbls of lead cement at 14.2 ppg. Halliburton pumped 269 bbls of tail cement at 15.6 ppg. The plug was dropped, and displaced with 304 bbls of water. The plug bumped the float collar at 04:31pm on 03/28/2013. Pressure held. The float held when the pressure was released. 4.0 bbls of water bled back.

0206835090 2013-03-14 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the 9 5/8 intermediate casing that was set to 1491. There were delays in the job as the wrong shoe joint parts were ordered and they needed new ones to be ordered to run the casing. There was also a delay as there was a miscommunication between Halliburton and Chesapeake as to when the required callout time was. In total 16 barrels of cement came back to surface with full returns through the job. Cement was scaled at 14.7ppg at end of the job, target was 15.6.
0366967212 2015-12-07 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

2078849353 2016-02-25 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #3H well is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the tank battery.

The equipment on the pad consists of the following: 6 Storage tanks 1 Vapor recovery unit (VRU) 3 Gas processing units (GPU) 1 Vertical separator 3 Sand separators 1 Thermal electric generator 1 Heat treater 3 Producing wells Site fenced and man doors locked
