0156550972 2017-07-12 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access entrance and the tanks. I did not locate an identification sign at the well head and I requested to Jody Jones of CHK on 07/12/2017 to have an identification sign posted at the well. The #3H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 4 producing wells

1 Low pressure vertical separator 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater The access road was in good repair.

0298079688 2017-09-06 Production Wells No Violations I conducted a status check of the well after my request to the operator on 07/12/17 to have an identification sign posted at the well. An identification sign was present during this inspection. No other changes have been made to the well or tank battery since my last inspection.
0531647219 2013-03-07 Administrative Inspection No Violations I was onsite to pickup the paperwork from the surface cement job. The rig is currently preparing to start drilling the intermediate casing hole. There are no violations at this time.
1029892658 2013-03-19 CM No Violations I was onsite to witness the cementing of the 5.5 production casing set to 13569. The casing was reciprocated and rotated at 10rpm until it torqued up and stalled out at 12k pounds, 240 barrels into the tail cement pumping. The plug was seated in the float shoe at 4pm and the well shut in for cement to set. No cement was returned to surface as it while it was expected to. The scaled mud weight was 11.8 ppg when the plug was bumped, suggesting it was a mudpush/drilling mud mix. This states that the cement has made it up into the 9 5/8 casing set to 1828. There are no violations at this time.
1362681020 2013-03-05 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the 13 3/8 surface casing set at 980 feet. There was a delay in running the casing due to an issue with am improper float shoe equipment. The commenced and there were complete returns through the job. A total of 50 barrels of cement returned to surface.

1387084378 2013-07-15 Production Wells No Violations I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completion of the production facilities for the four wells.

1629583925 2015-10-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1794667408 2013-04-15 Drill / Deepen / Reopen No Violations I was onsite to get updated GPS coordinates for the wells on the Harvey pad. There are 4 drilled wells on the pad, 1 well was previously drilled and is producing. There is production equipment onsite including a 6 400 barrel production tank battery surrounded by a steel dike. The wells appear to be static and the site is being prepared to be hydraulically fractured in the near future. A Grady Rentals crew was onsite laying down containment for the well pad. Surrounding the well pad is new erosion controls which have been recently installed. There are no site violations at this time.
1901422190 2013-06-06 Production Wells No Violations I was onsite to check the status of these wells. The wells have been drilled, hydraulically fractured and fully prepared for production. Currently crews are onsite preparing the flow lines for the wells to the production facilities onsite. The wells are currently shut in and the annulus and wellheads show not recordable pressures at this time. There is a gas sales line for this location so that gas will be sold when the wells are in production. There are no violations at this time for this location and these wells.
1924661796 2013-05-13 Drill / Deepen / Reopen No Violations A workover rig was on location to prepare the wells for production following the fracturing of the wells. Production equipment is set up on location and pipelines are on the location for production of the wells. A crew member said that they would be working on location several more days until all the wells are completed, following this a crew will come onsite and install the wellheads for the wells.
2069076197 2013-03-12 Drill / Deepen / Reopen No Violations I was onsite to check the status of Nomac 72 drilling the 8.5 hole for the 5.5 production casing. They were currently drilling ahead at 6200 and just switched out their hammer bit for a trichome bit. The Salina formation was causing excessive wear on the hammer bits.
