0077272950 2015-12-04 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0484588856 2013-03-04 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision rathole has drilled a 26 hole to a depth of 12 and then drilled a 24 to a total depth of 120. Precision Rathole ran 116 of 20 conductor casing and cemented the casing with 240 sacks of Class A cement ( 2% CaCl, .5 pound per sack cello flake, and 0.4% C41P ). The cement had a density of 15.6 pounds per gallon, yield of 1.18 cubic feet per sack, and used 5.24 gallons of mix water per sack. There was good circulation throughout the job and there was 10 barrels of cement circulated to surface. I walked the well site inside and outside the chain link fence and found no violations at this time. Gene Belsom (consultant) advised that Nomac 310 would be moving to this well site after they were finished on the Jeffery pad.
1316045983 2016-02-25 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #1H well is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the tank battery.

The equipment on the pad consists of the following: 6 Storage tanks 1 Vapor recovery unit (VRU) 2 Gas processing units (GPU) 1 Vertical separator 2 Sand separators 1 Thermal electric generator 1 Heat treater 2 Producing wells Site fenced and man doors locked
