0294656929 2015-12-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0370889362 2013-03-28 Drill / Deepen / Reopen No Violations I returned to the site to witness the cementing of the 13 3/8 surface casing. I arrived on site at approximately 3:30pm and observed that they were getting ready for the pre-job safety meeting. We had the safety meeting and O-TEX finished rigging up the cement head. Yost Rig # 6 ran 709 of 13 3/8 casing and O-TEX cemented the casing with 640 sacks of Class A 2% CaCl, .27% cello flake, and 0.4% C-41P. The cement had a density of 15.6, yield of 1.2, and used 5.25 gallons per sack of mix water. O-TEX pumped 137.78 barrels of cement and displaced the plug with 103 barrels of freshwater. There was good circulation throughout the job and approximately barrels of cement was circulated to surface.
0801933675 2016-02-18 Preliminary Restoration No Violations

I was on location to conduct a preliminary restoration inspection. The #6H well is in production. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of: 8 Storage tanks 6 Producing wells 6 Sand separators 6 Gas processing Units 1 Vapor Recovery Unit The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface and is in good repair. The access gate to the lease road was not locked. The lease road is stable and drivable.

1364503617 2013-03-28 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. When I arrived on location Yost Drilling Rig # 6 had 5 or 6 joints of casing and the landing joint / mandrel wellhead left to run. They ran the casing and tried to land the casing in the Sweco Wellhead, but the mandrel wouldnt land into the wellhead properly. The rig tried to get the mandrel to land in the wellhead several times before contacting Sweco to send a company representative to the location to see what the problem was with the wellhead. Sweco advised that they would have somebody enroute to the location as soon as possible. Since it was going to be awhile before the Sweco personel would be on site, I left the location to witness the cementing of the 20 conductor casing on the Putnam #1 and #5 wells. Before leaving the location, I advised Zach Grim (company man) that I would return to the site later to witness the cementing of the 13 3/8 casing.
1508426082 2017-10-16 Production Wells No Violations The well is in production. No changes have been made to the well or production equipment since my last inspection. The pad consists of six production wells. No violations were noted at the time of inspection.
