0634910149 2017-10-16 Production Wells No Violations The well is in production. No changes have been made to the well or production equipment since my last inspection. The pad consists of six production wells. No violations were noted at the time of inspection.
1129551660 2015-12-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1358365040 2013-01-16 Drill / Deepen / Reopen No Violations I was on site to check on the status of Precision Drilling. My inspection revealed that they have drilled the conductor hole to a total depth of 120 and ran 120 of 20 casing into the wellbore on the #10 well. They advised me that they have other conductor holes on this pad and they will drill those holes and run casing before contacting the cementers. They have two rigs drilling at this time on the # 6 and #8 wells. They also advised that Chesapeake has decided not to drill the # 3 and # 5 wells at this time. They should be finished drilling and running casing by the end of this week and could possibly be ready to cement the conductor casings sometime this weekend.
1723939556 2017-10-16 Production Wells No Violations The well is in production and is equipped with a plunger-lift assembly. No changes have been made to the pad since my last inspection. No violations were observed at the time of inspection. The 13.375 annular pressure displayed a reading of zero psi.

Other production equipment on the well pad consisted of: 8 Storage tanks 6 Producing wells 6 Sand separators 6 Gas processing Units 1 Vapor Recovery Unit The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface and is in good repair. The access gate to the lease road was not locked. The lease road is stable and drivable.
