-Tex was the cementing contractor. There was good fluid circulation throughout the cementing operation. O-Tex pumped a 50 bbl barite spacer to recover the oil based mud and condition the hole. O-Tex then pumped 298 bbls of lead cement that was a 50/50 Poz Premium H cement. The lead cement was mudscaled at 13.3 and 13.5 lbs per gallon (ppg). O-Tex the pumped 570 bbls of tail cement that was a Premium H cement. The tail cement was mudscaled at 15.3 ppg. The plug was dropped, and displaced with 400 bbls of water. The plug bumped the float collar at 2:36 A.M. on 02/6/2015. Pressure held. The float held when the pressure was released and 5 bbls of water bled back. The permit was in the company mans trailer. There were multiple fire extinguishers around the location while the job was being performed. No violations found.

0288013608 2015-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.

1288079199 2018-11-08 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of 50 psi, and the intermediate/production annular gauge displayed 407 psi. I performed a walk-around inspection of the production pad and no violations were observed at the time of inspection.
1344618648 2015-01-27 Casing String Witnessed No Violations I went to the site to witness the cementing of the surface casing. The drilling contractor for Chesapeake is Nomac Rig 77. Nomac drilled a 17.5 diameter hole to 957. There were no issues while drilling the surface hole. The 13.375 OD surface casing was set at 957 with a 27 landing joint. A total of 8 centralizers were installed on the surface casing string. O-Tex was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 16 lbs per gallon (ppg), O-Tex adjusted the cement density, and pumped 179.5 bbls of cement slurry at 15.6 ppg. Cement was mudscaled two other times during the job and both times the cement mudscaled at 15.6 ppg. Cement returns to surface were observed. An estimated 43 bbls of cement returned to surface. The cement returns were mudscaled at 15 ppg. The plug was displaced with 138 bbls of water. The plug bumped the float collar and O-Tex pressured up on the casing and held the pressure for 5 minutes. The float held and approximately 0.5 bbls of water bled back when the pressure was released. The permit was in the company mans trailer. There were multiple fire extinguishers present during the cement job. The location was in stable condition. No violations found.

1423066408 2015-01-28 Drill / Deepen / Reopen No Violations On 1/14/2014, Nomac Rig 77 drilled a 12.25 hole to a depth of 1897.20 and installed 1893 of 9.625 40 lb./ft. intermediate casing. O-Tex Cementing was the cementing contractor. O-Tex pumped 30 barrels of Gel spacer, followed by 10 barrels of water, follow by 670 sacks (144.39 bbl.) of Premium H 15.8 lb/gal cement. The plug was displaced with 136.9 barrels of water. Plug landed incresed pressure to 1580 psi., floats held and there was 16 barrels of 15.0 lb/gal of cement returned to surface.
1423288407 2018-04-23 Production Wells No Violations I conducted a status check of the pad. The pad is equipped with eight producing wells, and partially drilled wells with conductor casing (# 22624 & 22623). The surf/int pressure gauge displayed a reading 50 psi, and the int/prod displayed 501 psi. The tank battery consisted of six -400 bbl production tanks. Other equipment on the pad consisted of the following.




The access road was stable. No violations were observed at the time of inspection.

1424352290 2015-02-03 Drill / Deepen / Reopen No Violations Nomac rig 77 is drilling a 8.25 hole at a depth of 15866. Permits and conditions are posted in the company trailer. I performed a walk around the rig I did not notice any violations at this time.
1537805745 2018-09-17 Production Wells No Violations No equipment changes have been made to the pad since my last inspection. The surface-intermediate annular gauge displayed a reading of 54 psi, and the intermediate/production annular gauge displayed 464 psi. I performed a walk-around inspection of the pad, and no violations were observed at this time.
