0170808909 2013-03-04 Drill / Deepen / Reopen No Violations I was on site to check the status of Patterson Rig # 328. My inspection revealed that Patterson Rig # 328 is moving onto location. I was advised that they should be rigged up and spudded by this weekend. I walked the area outside the chain link fence and found no violations at this time.
1363047200 2013-02-26 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole has drilled a 24 hole to a total depth of 115 and ran 20 casing to a depth of 115. O-TEX cemented the casing with 175 sacks of Class A 2% CaCl; .5 pound per sack cello-flake; .4% C41P. The cement had a density of 15.6 pounds per gallon and a yield of 1.2 cubic feet per sack. O-TEX pumped 36.78 barrels of cement slurry and approximately 6 barrels of cement was circulated to surface. NOTE: O-TEX had trouble breaking circulation before they started mixing the cement.
1499970405 2017-07-12 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #1H well is in production and is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 4 producing wells

1 Low pressure vertical separator 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater The lease road was stable and in good repair. No violations were noted at the time of inspection.

1956053855 2013-03-11 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Drilling Rig# 328 has drilled a 17.5 hole to a total depth of 460 and ran 13 3/8 casing to a depth of 445. They installed four bowspring centralizers on joints 1,2,5,8 in the casing string. O-TEX cemented the casing with 410 sacks of Class A 2% CaCl; .27% cello-flake; and .4% C41P. O-TEX pumped 87.6 barrels of cement slurry and displaced the plug with 62.3 barrels of water. There was good circulation throughout the job and there was cement circulated to surface. My inspection revealed no violations at this time.
2094305046 2016-02-19 Preliminary Restoration No Violations I was on location for a status check. My inspection found the access road in good repair. Identification signs are posted at the access road entrance, well and storage tanks. The pad is fenced and all man doors were locked. The #1H is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the storage tanks.

2135502159 2015-10-20 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
