0864470519 2017-07-10 Production Wells No Violations I conducted a status check of this well in response to complaint #5646 regarding excessive noise coming from the compressors on the location. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #6H well is in production and is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. There were several inches of standing water inside of the tank battery. I test the standing water using a low-range Quantab which concluded in a reading under 30 ppm.The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 6 producing wells

1 Low pressure vertical separator located inside of dike 6 sand separators 6 gas processing units 1 vapor recovery unit 1 heat treater 3 compressor stations; one located on the south side of the pad with, and two located on the east side of the pad. The two compressors located on the east side of the pad did not have any sound barriers around the units. I will contact Chesapeake and request that sound barriers be installed around the units to mitigate the noise issue.

1371683422 2015-10-26 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1397737222 2014-04-08 Production Wells No Violations I inspected the well site and found the well head to be constructed with a Christmas tree valve and piping. There are six wells at this site. The condensate tanks and piping runs were all intact with no leaks. The spray lined steel containment around the condensate tanks had rain water in it with no visible contamination. The diesel power unit was found to have excess oil in the spill containment under the engine. The pre-heater unit had no visible issues. While I was finishing my inspection, the contractor was there to clean up the oil under the diesel engine. No oil reached the ground. The lease road is stable condition and the site is well secured with two locked gates. No violations were found.
1466489486 2018-04-25 Production Wells No Violations I conducted a follow-up inspection to complaint #5646 regarding noise from the pad. The surface/intermediate and intermediate/production annular gauges displayed readings of zero/70 psi. No anomalies were observed on the pad. Sound barriers were still installed around the compressor units. The complaint will be closed.
1489955401 2017-07-26 Production Wells No Violations I followed-up in response to complaint #5646 regarding noise issues on the pad. Sound barriers had been installed and were present around the two compressors on the east side of the pad. No other changes were made to the well or pad since my last inspection.
