0392068623 2016-02-18 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was not locked. The access road was in good repair. The #1H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence. The man doors were not locked, but Chesapeake Exploration personnel were on site at the time of inspection.

The equipment on the pad consists of the following: 8- 400 bbl production tanks with one low pressure vertical separator 6 Producing wells 6 Sand separators 6 Gas Processing Units 1 Heat Treater 1 Vapor Recovery Unit

1266457620 2015-11-09 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1355275338 2012-12-11 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Yost Rig # 6 has drilled a 17 1/2 hole to a total depth of 591. They ran 591 of 13 3/8 (54.5# per foot) casing with 6 centralizers on joints 1,2,3,6,9,12 . O-TEX cemented the casing with 580 sacks of Class H cement with 2% CaCl; .27% Cello Flake; and .4% C-41P. They pumped a 25 barrel gel spacer and a 10 barrel pre-flush ahead of the cement. They pumped 123.96 barrels of cement and displaced the plug with 84 barrels of freshwater. O-TEX bumped the plug at 11:31pm with 350 PSI. There was good circulation throughout the job and there was 30 barrels of cement circulated to surface. They conducted casing integrity test at 920 PSI for 5 minutes; test approved. No violations at this time.
