0658606653 2012-08-15 Drill / Deepen / Reopen No Violations I was on site to check the status of Nomac Drilling Rig # 43. My inspection revealed that Nomac Rig # 43 has skidded to this well and is rigging up and should spud the well either late tonight or early tomorrow morning. Larry Williamson (company man) advised that they should be ready to run and cement the 13 3/8 surface casing sometime tomorrow. He also advised that he would call me before they spud the 17.5 hole. My inspection also revealed no violations at this time.
1148128090 2012-08-19 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. My inspection revealed that Nomac Drilling Rig # 43 has drilled a 12.25 hole to a depth of 2226. Wyoming Casing Crew ran 2017 of 9 5/8 casing with 19 centralizers installed on the casing string (on joints 1,2,3,6,9,12,15,18,21,24,27,30,33,36,39,42,45,48,51). Schlumberger cemented the casing with 781 sacks of ClassA cement (10% salt; .125 pound per sack D130; .2% D046; .15% D1530) with a density of 15.6 pounds per gallon and a yield of 1.24 cubic feet per sack. There was good circulation throughout the job and approximately 40 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
1614273235 2012-08-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Nomac Drilling Rig #43 has drilled the 17.5 hole to a depth of 580. Wyoming Casing Crew ran 570 of 13 3/8 casing with six centralizers installed on the casing string (on joints 1,2,3,6,9,12). Schlumberger cemented the casing with 524 sacks of ClassA cement (2% S001 CaCl; .2% D046; .125 pound per sack D130 flake) with a density of 15.6 pound per gallon and a yield of 1.2 cubic feet per sack. There was good circulation throughout the job and there was approximately 23 barrels of cement circulated to surface. My inspection revealed no violations at this time.
1840718817 2015-11-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
