0633294345 2015-09-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1283478691 2015-01-15 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:

1385788370 2012-07-20 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. I received a phone call from the Larry Kidd (Rig Consultant) at 11:06 pm (7/19/12) and he advised that they should be finished running the 13 3/8 surface casing around 6:00am tomorrow morning (7/20/12) and O-Tex Pumping should be on site at that time (6:00am). I arrived on site at 6:45am and observed that Patterson Rig # 328 and Wyoming Casing Crew was still running the 13 3/8 surfce casing. O-TEX Pumping was on site and in the process of moving trucks and rigging up to cement. Patterson Rig 328 crew and Wyoming Casing finished running the 13 3/8 surface casing and rigged down the casing crew . O-TEX rigged up the cement trucks and installed the cement head on the casing . A safety meeting was held and then they started the cement job . O-TEX Pumping cemented the 485 of 13 3/8 casing with 450 sacks of Class H cement (2% CaCl , .27% Cello Flake,and .4% C-41P defoamer ; 15.6 pounds per gallon density and a yield of 1.18 cubic feet per sack). There was good circulation throughout the job and approximately 35 barrels of cement returns to surface. Bumped plug at 11:19 am and floats held.
1475117427 2016-02-22 Production Wells No Violations I conducted a status check inspection of this Utica Shale well pad. My inspection found the lease road gate present and locked. The lease road and apron are in good repair. The pad is fenced and the entrance gate and all man doors on the perimeter of the pad were locked. The #1H well is in production, equipped with a plunger lift system, and the gauge on the 2.875 tubing indicates a pressure of 338psi. The 5.5production casing wing valve indicates a pressure of 115psi. The well is choked back and producing at a pressure 211psi to the surface production equipment. Well #s 3H (PN# 22075), 5H (PN# 22151), 6H (PN# 22123), 8H (PN# 22124)and 10H (PN# 22125) are also in production and producing with the aid of plunger lift systems. The proper IDs are posted at the entrance and at the wellheads and tank battery. There is a one (1) acre, freshwater impoundment located to the north of the pad. The impoundment is lined, fenced, locked and identified. The pad and impoundment area are equipped with video surveillance.
