-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The lead slurry was mudscaled, and Schlumberger pumped 152 bbls of lead cement at 13.5 pounds per gallon (ppg), and 491 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 303.5 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 7:36am on 08/06/2013. The plug landing pressure was 2600 psi, and it was raised to 3200 psi and held for 5 minutes. Pressure held. The float held when the pressure was released, and 3.5 bbls of water bled back. It was estimated that 50 bbls of the Mudpush spacer returned to surface.

0218153045 2016-03-10 Preliminary Restoration No Violations

There are concrete jersey barriers protecting the wellhead and production equipment from vehicles. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

0364693737 2015-09-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of two 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0497785175 2013-07-17 Drill / Deepen / Reopen No Violations I stopped by to check on the site preparations for the move-in of the drilling rig. I found that preparations were underway, but no rig equipment was on site. Some secondary containment was staged, and a number of frac tanks and light plants were in place. I took GPS readings for the well and access road entry. A security guard was stationed along the access road.
