0422815301 2012-07-30 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. My inspection revealed that Patterson Rig # 329 and Wyoming Casing was still running the 9 5/8 casing when I arrived on site. They had approximately ten joints of casing, the landing nipple and the landing joint left to run before they were ready to start cementing the casing. While running the casing, they installed 15 bow-spring type centralizers on the casing string (on joints 1,2,3,6,9,12,15,18,21,24,27,30,33,36,39). Schlumberger cemented the casing with 671 sacks (146.9 barrels) of ClassA cement with a density of 15.6 pounds per gallon and a yield of 1.24 cubic feet per sack. The additives in the cement blend were 10% D44 (salt), .2% D46 (antifoam), .15% D153, and .125% D130 (lost circulation material). Schlumberger pumped 133 barrels of displacement to land the plug. There was good circulation throughout the entire job and 40 barrels of cement was circulated to surface. After the plug was landed, Schlumberger pumped 1000 PSI on the casing and held it for five minutes with no pressure loss.
1342156403 2012-07-12 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. Express Energy Services drilled a 26 hole to a to a total depth of 118 and ran 118 of new 20, 94 pound per foot conductor casing. Allied Oil and Gas Services cemented the casing with 277 sacks of Class A cement (2% CaCl and .25 #/sack Floseal) and circulated approximately 5 barrels of cement to surface.
1343702251 2012-07-27 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Rig 329 has drilled the 17.5 hole to a depth of 574 and ran 13 3/8 casing to a depth of 574. They installed six centralizers on the casing string (joints 1,2,3,6,9,12). Schlumberger cemented the casing with 464 sacks of Class A with a weight of 15.6 pounds per gallon and a yield of 1.2 cubic feet per sack. Cement additives were .2% D46 (antifoam), .125% D130 (lost circulation material), and 2% S001 (CaCl). Schlumberger pumped 81.3 barrels of displacement to bump the plug at 3:10pm. There was good circulation throughout the entire job and 15 barrels of cement was circulated to surface.
1934989243 2012-07-10 Drill / Deepen / Reopen No Violations DNR Inspector Shawn Murphy and I were on site to the the status of Express Energy Services. Our inspection revealed that they have the tin horn (cellar) set and are starting the 26 conductor hole. The rat-hole rig operator advised that they should finish this conductor hole today and start the hole for the Slates 8H either later tonight or early tomorrow morning. No violations were noted during this inspection.
2072657876 2015-12-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
