0813430143 2013-12-30 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Drilling Rig 74 has drilled a 17.5 inch hole to a total depth of 598 feet and ran 588 feet of 13 3/8 inch casing (54.5 pound per foot casing and inside diameter of 12.615 inches). They installed 6 bowspring centralizers on joints 1,2,5,8,11, and 14. Schlumberger cemented the casing with 526 sacks of Class A (D901) 2% D154, 0.5% B248, 0.2% B160A, 0.2% D046, 1% S001, and .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.234 gallons of mix water per sack. They pumped 113 barrels of cement slurry, 84 barrels of displacement, and there was 25 barrels of cement returns. They landed the plug at 5:53am with 272 PSI; bumped pressure up to 817 PSI and conducted the casing test. The floats held and there was 1/2 barrel flowback when the pressure was released. The casing test was approved. My inspection revealed no violations at this time. NOTE: Had to wait on the Sweco wellhead hand to arrive on location so they could land the casing in the wellhead. He arrived on site at approximately 02:30am.
0920522754 2013-12-31 Drill / Deepen / Reopen No Violations I was on site to check the status of Nomac Drilling Rig # 74. My inspection revealed that Nomac Drilling Rig # 74 is drilling the 12 1/4 inch intermediate hole at 1262 feet. My inspection also revealed no violations at this time.
0934222177 2015-12-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1388397696 2013-12-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision drilling has set 30 inch casing to a depth of 20 feet. They drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch casing (94 pound per foot/ inside diameter of 19.124 inches). Allied cemented the casing with 309 sacks of Class A 2% CaCl and 1/2 pound per sack flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. They pumped 65 barrels of cement slurry, 39 barrels of displacement, and there was 8 barrels of cement returns to surface. My inspection revealed no violations at this time.
