0171565468 2014-04-16 Casing String Witnessed No Violations I was on location at noon to inspect cementing operations. Wyoming Casing was running the last 1000 of 5.5 production casing into the hole. They ran a total of 367 joints of casing with a float shoe on bottom and a float collar at 15,826. Black Hawk and O-Tex Pumping were on location waiting on the casing run. Wyoming Casing landed the pipe on bottom and Nomac #73 stabbed into the casing with the top drive and began circulating the hole. The pipe was rotated and reciprocated for an hour while circulating to clean up the hole. Nomac lifted Black Hawks rotating cement head to the floor and began rigging it up to the casing. I left location at 4:30pm. No violations.
0258940032 2014-04-04 Casing String Witnessed No Violations I was on location at 9:10pm to witness the cementing of the 13.375 surface casing. Nomac drilled a 17.5 hole to a depth of 759 and Wyoming Casing ran 749 of surface casing into the hole. O-Tex Pumping cemented the casing with 149 barrels of 15.6lb/gal cement slurry. 23 barrels of cement circulated to the cellar. See casing ticket for more details on this job. No violations.
0858351972 2014-04-08 Drill / Deepen / Reopen No Violations On location with supervisor Tom Hill. Nomac 73 is drilling the 8.75 curve section at a depth of 5,766. The mud up point is 6,884 and the kick off point for the curve will be 6,934. We spoke with the company man and inspected operations. No violations.
0957258114 2014-04-03 Drill / Deepen / Reopen No Violations I was on location to check the status of Nomac Rig #73. There are 2 producing wells on this pad that are shut-in and secured. They are currently rigging up over the #1H and will be the first of 4 more laterals to be drilled on this pad. The location is in good condition and all equipment is on containment. They will spud the 17.5 surface hole after midnight. No violations.
1182359716 2015-12-04 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1409764327 2014-04-14 Drill / Deepen / Reopen No Violations On location to check the drilling status of Nomac Rig #73. They are currently drilling the 8.5 lateral at a depth of 14,489. The rig will reach a total measured depth of 15,900 and cement 5.5 production casing on Wednesday. I inspected the 5.5 casing on the racks. It is new, P-110, 5CT, weighing 20lbs/ft. and made in the USA. All equipment is on containment. No violations.
1546919163 2017-08-30 Production Wells No Violations

I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #10H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The equipment on the pad consists of the following: 6 producing wells

1 Low pressure vertical separator 1 heat treater 6 sand separators 6 gas processing units 1 combustor unit
