0535431509 2015-04-28 Production Wells No Violations I meet with the Federal EPA at this location. They performed an Air Quality Inspection of the well and tank battery. I performed a status check of the tank battery and well head. ID signs are posted. No violations were noted at this time.
0844831222 2016-02-22 Production Wells No Violations I was on location for a status check. My inspection found the access road gate is locked and the access road is in good repair. Identification signs are posted at the access road entrance, well and storage tanks. The pad is fenced and all man doors were locked. The #3H M is in production. The well is equipped with a plunger lift assembly. There are two steel dikes. One contains six storage tanks and the other contains two storage tanks.

The equipment on the pad consists of the following: 8 Storage tanks 3 Sand separators 3 Gas processing units (GPU) 2 Vertical separators 1 Vapor recovery unit (VRU) 4 Production wells on the pad Site fenced and man doors locked

0980080630 2012-06-25 Preliminary Restoration No Violations My inspection revealed that the Preliminary Restoration can be approved. All the erosion and sedimentation controls are functional and the vegetation is adequate to prevent substantial erosion and sedimentation.
1021767511 2015-12-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. On the outside of the well pad there was a enclosed, chain link fence with three strand barb containing two 400 BBL production tanks. The lease road is stable and drivable. Preliminary restoration passed.
