0057837194 2018-05-03 Production Wells No Violations I conducted a compliance-notice follow-up to this well for contamination around the well head. I was also on site in response to compliance number 2198 regarding lease road erosion. All contamination has been removed from the well head. The area around the well contained gravel and was diked. The compliance notice can be closed. The lease road had been levelled and lined with gravel to minimize washout. No erosional/sedimentation issues were visible at the time of inspection. No violations were observed at the time of inspection.

0370151255 2017-05-25 Production Wells No Violations I conducted and inspection of the well site with DOGRM Regional Supervisor Tom Hill in response to the complaint #5198, involving erosion taking place on the lease access road. Erosion of the lease road was evident with washout and sedimentation occurring in the adjacent farm field. Erosional gullies were present along the access road. I contact Tim McConaha of Enervest on 05/31/2017 to have the issue addressed as soon as possible.

0618131995 2018-03-07 Production Wells Violations Noted I was on site with DOGRM District Supervisor Stephen Ochs in response to complaint #5198 regarding lease road erosion. An area of washout was still visible near the lease road. I spoke to Bob Whaley of Enervest about my ongoing request to have the lease road mitigated, who informed me that due to wet soil conditions they cannot work on the site at this time, but will as soon as weather permits the ground to dry. I inspected the well, and an area of oil (mixed with water) was present around the well head measuring approximately 3.5 feet x 1.5 feet. Absorbent peat was also present in the contaminated area. I spoke to Bob Whaley about the contamination, and it was stated the contamination will be removed as soon as possible. This compliance notice will be closed once the contamination around the well head is removed.

0882292577 2015-12-10 Production Wells No Violations I conducted and inspection of the well site with DOGRM inspector, Ashley Raff with regards to the complaint #5198, involving erosion taking place on the lease access road. The inspection revealed that the lease access to the wellhead was eroding along the north side and washing into the adjacent farm field. The landowner has requested that the access be repaired. I spoke with the landowner, Tom Borton about installing best management practices to repair the erosion on the lease access. I will contact Enervest with regards to implementation of the best management practice.

Toolpusher: Eric Poulson advised that they should be ready to set surface tomorrow evening or early Wednesday morning. No violations at this time.
1146512067 2006-05-01 Drill / Deepen / Reopen No Violations Inspection revealed that CapStar drilling has finished drilling the well and has moved off location . Inspection also noted that the drilling pit fencing is missing on the south and west sides of the drilling pits . I called Jim Kramer (Belden&Blake) and advised that the fencing must be installed immediately.

1912887117 2018-03-15 Production Wells No Violations I conducted a compliance notice-follow up. The notice was issues regarding contamination around the well head. I spoke to Bob Whaley of Enervest the day the compliance notice was issues. He stated that they will stabilize the area and contain the product, but excavation cannot be completed at this time due to accessibility of the well head and weather conditions. At the time of my inspection, all visible product was contained, and absorbent peat was present around the well head. No mobile product was visible at the time of inspection. I will close the compliance notice once all contamination has been removed.
