0047898299 2014-12-08 Production Wells No Violations I was on site to follow-up on a tank battery explosion and fire that occurred at this location on 12/06/14 that also affected PN#s 21909 & 21759. Clean-up is on going and the damaged tanks and equipment are going to be cut up to get them out of the stand of trees. They have emptied the brine vault and are going to steam clean inside the vault, collapse the sides, and properly fill and bury the debris. This clean-up continues.
0282090747 2014-12-11 Production Wells No Violations I was on site to follow-up on a tank battery explosion and fire that occurred at this location on 12/06/14 and also affected PN#s 21909 & 21759. The are currently rebuilding the dike and tank battery. New tanks will be installed next week. Old tanks and equipment are being cut up and placed in a roll-off box for disposal. I walked the location looking for any affected areas not yet addressed and found none. This clean-up continues.
0637333073 2017-02-22 Production Wells No Violations The well is equipped with an electrically-driven pumpjack and the production valves were in the open position. The tank battery shares production equipment with PN# 21909 and consist of two 210 bbl production tanks, one 50 bbl brine tank, two vertical separators, and two meter runs with current charts. The dike was in good repair The load lines were secured with locks and plugs. The lease road was in good repair.
0961856367 2014-12-06 Production Wells No Violations I received a call from Troy Wells at 8:30am about an explosion and fire at this tank battery that occurred around 1:00am. I was on location at 10:30am to assess the situation. I met with Troy Wells (Enervest) and reviewed the location. Sutton Pump was on location and steaming the areas affected by oil and J Hawk Water Service was collecting the oil. 2--019-2-1909-00--019-2-1735-00--019-2-1759-00-00) also caught on fire at the flange and melted the chart box. I spoke with Ralph Castellucci (Fire Chief, Station #7) and he said that burned until the well was shut-
1388548424 2015-01-05 Production Wells No Violations I was on location with Regional Supervisor Tom Hill to inspect the condition of this location following a tank battery explosion on 12/06/14. We noticed an electrical arcing sound created by the electrified fence at the gated entrance to the lease road. We walked back to location and found that the tank battery for this well and PN# 21909 had been rebuilt with 2 new, 210 barrel tanks. Enervest had installed grounding wires on the equipment and equipped the tanks with spark arrestors and charcoal filters. The dike has been properly rebuilt and the 2 original, vertical separators were located inside the battery. The burned flange and meter run for PN# 21759 has been repaired and the original 210 barrel tank and vertical separator are inside the diked battery. We found the electrified fence 110 to the east of the battery at the edge of the creek was also arcing. It is unknown at this time if the arcing, electrified fence could have caused the explosion. I will continue to monitor the location through the spring for any issues. No violations.
