0414882593 2016-04-16 Production Wells No Violations I followed up on site to observe the flow/discharge cleanup. No personnel were on site during my inspection. A leak in the flowline near the wellhead was found and repaired. A trench was excavated near the wellhead at the source of the leak and left exposed observation. I did not observe any residual oil or sheen. Patches of oil residue were present in the open trough located downhill from the well and parallel to the stream, and clean oil soak pads were present. I walked the course of the stream and did not locate any sheen or residue.
0564606084 2017-08-02 Production Wells No Violations I conducted a status check of the well. The well is equipped with an electrically-driven pumpjack. An area of absorb at peat was around the wellhead. I did not locate any active leaks. The tank battery consist of one 100 bbl production tank, one underground cement brine vault, one vertical separator, and one meter run with a current chart. The lease road was in good repair.
1240734624 2016-03-28 Production Wells No Violations I followed up on site after DOGRM Terry Whitehair received a call from the landowner stating that more sheen was identified in the stream along his property. The area received heavy rain during the previous night. On site were Charlie Youngen from Enervest, and Wade Balser of the OEPA. Residual amounts of oil and sheen were present in the water adjacent form the trench. The access point appears to be the result of contamination leaching through the stream embankment, adjacent to the trench excavated parallel to the stream on 03/24/2016. I walked north, downstream from the access point, approximately 200 feet. Pockets of sheen could be identified in the water, but I did not locate any free product downstream. Several booms were deployed along the stream. A siphon dam was being installed in the stream just north of the access point. The plan of action is for Enervest to continue monitoring and cleaning any contamination. Another trench is planned to be excavated uphill and closer to the wellhead on 03/28/2016 to try to locate the source of the contamination.

1462899835 2016-03-24 Production Wells No Violations I received a call from Charlie Youngen from Enervest at 12:30 PM on 03/24/2016. He reported that Enervest was notified by the landowner who noticed oil sheen in the stream along his property. I responded on 03/24/2016. On site were Charlie Youngen from Enervest, and DOGRM Terry Whitehair. The OEPA was notified by Enervest. We did not locate any residual sheen while on site. According to Enervest, the initial source of the sheen was thought to stem from an undocumented release occurring approximately two years ago, which had been responded to and was cleaned up at the time. A trench was excavated east and downhill from the well, and parallel to the stream in an effort to try to determine a source of contamination. Excavation of the trench appeared to expose the buried pit liners that were used during drilling of the well. The excavated trench was slowly infiltrated with water and small amounts of oil. The oil appeared to be fresh. Oil soaks were deployed inside of the trench. Enervest shut-in the well for the weekend, and a 24-hour pressure test will be conducted on 03/28/2016 to rule out the possibility of a flowline leak. I conducted an inspection of the well and tank battery while on site. The well is equipped with an electrically-driven pumpjack. I could not locate any leaks at the surface around the well head during my inspection. The tank battery consists of one 210 bbl production tank, one vertical separator, on underground brine vault, and one meter run with a current 31-day chart. The tank battery is surrounded by a dike. Plugs were installed on the tanks production valves. Identification was posted at the well and tank battery
