0538557530 2015-03-27 Production Wells No Violations I performed a follow up inspection on this location. This location also shares the tank battery with the Hadorn #4 - 34021106 and # 5 - 3401921105. There is (2) two 210 barrel tanks, (2) in-ground concrete vaults and (2) two vertical separators. At the 2 valve on the back of the Carlson #1 1117 there is oil/fluid stained soil on the ground that needs to be removed an disposed of. ID signs are posted at the tank battery.
0828473253 2014-09-22 Production Wells No Violations Follow-up to Complaint #4800. I met on location with the landowner, Scott Galloway, to review some issues with the wells located here. I inspected the wellhead and it was equipped with an electric pumping unit. The landowner showed me several areas in the woods where the power line was lying on the ground. During the inspection, I noticed a bubbling in the lease road and determined the flow line from the #1 wellhead was leaking. There was less than a gallon of oil in the ruts on the lease road and it was gassing. I contacted John Smith (Green Gas) and he was sending someone out to shut the wells off. After about 40 minutes, Tim Smith (Green Gas) arrived, shut the wells off, and the leak ceased. He put down some sorbent pads to collect the oil. I showed Tim the issues I had found with the producing operations and explained to him what needed to be done to correct them. He will be out tomorrow with a mini excavator to repair the flow line, rebuild the containment dike, and address the leaks in the tank battery.
1223578112 2014-09-23 Production Wells No Violations Follow-up to complaint #4800. I was on location with Tim Smith (Green Gas) to inspect the repairing of the flow line. The line was exposed and repaired with a clamp and the affected soil was removed. I showed the excavator what I wanted done to rebuild the containment dike and Tim began working on the leaking fittings. I will check back next week that the proper repairs have been made.
1363550721 2013-03-06 Production Wells No Violations I conducted this inspection for Scott Galloway (landowner) because he has concerns about the annual production reports submitted by the operator. My inspection revealed that all the surface operating equipment required to operate the well is on site. I checked RBDMS production reports and found that the operator has reported 101 barrels of oil and 566 gas. This is a shared tank battery with permit number 1118 (Carson #2 well). Mr. Galloway is concerned because the operator has reported the same production for each well. I advised him that since it was a common tank battery and the production went into the same storage tanks and into the same gas sales line, the operator probably just split the annual production numbers in half and claimed that amount for each well. My inspection revealed no violations at this time.
1427580934 2015-03-26 Production Wells No Violations I inspected this well with DOGRM inspector John Fleming to follow up on Complaint # 4800. The tank battery contained production equipment for the Carson #s 1&2 (PN #s 21117, 21118) and the Hadorn # 4&5 (PN#s 21105, 21106) wells, and consisted of two 210 barrel tanks, two vertical separators, and two brine vaults. There was an oil leak at the Carson tank under the two-inch pick-up line which had been previously covered with Peat Sorb. The contaminated soil covered an area of three feet wide by four feet long. The well head was not investigated at this time. The operator is to dig and remove all contaminated areas for proper disposal. Green Gas was notified on 03/26/2015. They were to notify the proper personnel to remove and properly dispose of the contaminated soil. John Fleming or I will follow-up on the cleanup during the week of 03/30/2015.
1561592834 2014-09-16 Production Wells No Violations I was on location to investigate Complaint #4800 concerning a breached containment dike that the landowner says has been like this for 8 years and would like it repaired. My inspection of the tank battery found that it contained 2-210 barrel tanks, 2-vertical separators, 2-brine vaults, and 2-meter runs. The Carson #s 1 & 2 (PN#s 21117, 21118) produced to 1 set of the equipment and the other set of equipment was for production of the Hadorn #s 4 & 5 (PN#s 21105, 21106). The containment area measured approximately 36 X 54 and was well worn down on the north side where the flow lines entered the battery. There was a 12 wide cut in the dike on the north side that was made when a tank was replaced and never rebuilt. I noted a small leak at the outlet of the Hadorn tank and another at the end of the dump line on top of the brine vault. I left a message with Green Gas about my findings at the tank battery and to contact me to discuss the required repairs. I did not inspect the wellheads at this time. I contacted the landowner and will meet with him on 9/22/14 at 11:00am.

2124366543 2014-10-02 Production Wells No Violations Follow-up on Complaint #4800. The dike on the north side of the tank battery has been rebuilt and repaired. The dump line from the Carson#s 1&2 210 barrel tank has been repaired and is no longer leaking. The leak on the connection at the base of the Hadorn#s 4&5 210 barrel tank appears to have been repaired. I will continue to monitor this tank for leaks. I did not inspect the power lines at this time. This complaint continues.
