0406789089 2013-04-03 Plug / Plug Back No Violations I was on site to witness the remainder of the plug job. My inspection revealed that CHIPCO service rig and CHIPCO cement trucks pumped the Bottom 8 5/8 / Berea Plug from 1263-1063 ( 60 sacks of ClassA cement) and pumped the Surface Plug from 200-0 (60 sacks of ClassA cement). Circulated cement to surface on the surface plug and it stayed at ground level. No violations at this time.
0559375592 2013-04-01 Plug / Plug Back No Violations I was on site to check the status of CHIPCO service rig. My inspection revealed that they were blowing the well down so they could get the well ready for plugging. NOTE: There was a H2S gas odor present at the well head. I spoke with Chip and we decided that they would perforate the casing from 3678 to 3668 with ten shots. They are going to run tubing after they perforate the casing and spot the Clinton Plug and the Lime Plug thru tubing. They advised that Appalachian Well Services will be on site tomorrow morning to perforate the 4 1/2 production casing. No violations at this time.
1108136384 2011-10-20 Production Wells Violations Noted This well is currently shut in. There are no production lines or tank attached to this well. The records in our database show that this well last produced 249 thousand cubic feet of gas in 2000. The well is not properly identified. Post proper identification. Produce, plug, sell this well or apply for Temporary Inactive status. Failure to comply with this request may result in forfeiture of bond.
1209685987 2013-04-02 Plug / Plug Back No Violations I was on site to witness the pumping of the Clinton Plug (5285-5085) and the Lime Plug (3678-3378). Inspection revealed that CHIPCO cement trucks pumped 30 sacks of Class A cement for the Clinton Plug and 86 sacks of Class A cement for the Lime Plug. They advised that they should be ready to rip the production casing tomorrow morning sometime and the be ready to pump the Bottom 8 5/8 / Berea Plug and the surface plug sometime mid-day tomorrow. No violations at this time.

1799080278 2013-04-02 Plug / Plug Back No Violations I was on site to check on the status of CHIPCO service rig. My inspection revealed that Appalachian Well Services used 10 perf shots and shot the 4 1/2 production casing from 3678 to 3668 because there was a H2S gas odor at the wellhead when the service rig first arrived on location. The rig is going to run tubing back into the well today and pump the Clinton Plug (5285-5085) and then pump the Lime Plug ( 3678-3378). I will return to the site later today to witness these plug. No violations at this time.
