0578205140 2010-11-16 Production Wells Violations Noted On site to conduct an inspection at the request of John Rice (landowner) and Attorney Nathan Vaughn to see if well is in production. Inspection revealed that it appears that the well hasnt been pumped for quite sometime because the polish rod liner is tarnished, trees are growing against the pumping unit, and valves are rusted on the wellhead. Inspection of the tank battery revealed that there is pressure on the seperator and it appears that there is a domestic gas hook-up. Inspection of the tank battery also revealed no identification sign posted and the is black oil stained areas inside the tank battery SPCC dike. The buried cement brine storage tanks are full. I will contact the operator and advise that there are violations and the need corrected withing two weeks of receiving the notification.
1480699325 2016-11-22 Final Restoration No Violations I conducted a final restoration inspection on this well. The area of the well and tanks battery are vegetated with no active erosion. But the final restoration cannot be passed at this time. A pumpjack unit is still on site at these coordinates (40.52615, -81.19806). I contacted the number listed on record and the line is no longer working. The well owner Charles DeYarmon Jr. passed away in January 2016. The well is located on the property of Rev John P. Rice (Trust). *Lease Road Coordinates (40.52460, -81.19874).
1500554070 2017-05-05 Final Restoration No Violations I conducted a Final Restoration inspection on this well. The flowlines have been removed from the well site. The site is stabile and vegetated with no active erosions. The inground brine vaults and flowlines have been removed. The tank battery location is graded, seeded and mulched. The vegetation is just starting to grow. I will follow-up on a later date and if the vegetation is established the Final Restoration can be passed

The concrete brine vaults and flowlines have been removed from the tank battery location. The well and tank battery sites are graded and vegetated with no active erosion. The final restoration is passed.
